ADDENDUM TO SERVICE PROVIDER AGREEMENT

This Addendum to Service Provider Agreement is entered into by Legacy the School of Sport Sciences (“Legacy”) and ______(“Service Provider”) this 12 day of 5, 2017.

WHEREAS, Legacy and Service Provider executed an Agreement dated 11/27/17, for the purpose of Business & Finance Service ProviderAgreement(“Original Agreement”).

WHEREAS, the Original Agreement contemplates that, “1.1 PROCESS OVERVIEW: This section outlines the steps in the procurement process.

  • 11/27/17 Release of Request for Proposal (RFP)
  • 11/29/17 Deadline for questions concerning the RFP or services requested – 4:00 PM
  • 11/30/17 Responses given to any questions will be posted as an addendum at link above -2:00 PM
  • 12/1/17 Deadline for proposal submission – 1:30 PM
  • 11/30/17 Interviews with Business Office Support Provider (if needed)
  • 12/04/17 Board of Trustees meeting – contract awarded
  • 12/05/17 Contract commencement

“Business & Finance Services” may be provided or exchanged with Service Provider in connection with a financial product or service;

WHEREAS, the Legacy Collegiate Middle and High School for Careers in Health and Wellness requires Legacy to ensure, by contract, that Service Provider implements and maintains appropriate safeguards for Business & Finance Servicesprovided to Service Provider.

NOW THEREFORE, the parties agree to add the following requirements to their Original Agreement:

  • 12/5/17 Release of Request for Proposal (RFP)
  • 12/14/17 Deadline for questions concerning the RFP or services requested – 4:00 PM
  • 12/15/17 Responses given to any questions will be posted as an addendum at link above -2:00 PM
  • 12/19/17 Deadline for proposal submission – 2:00 PM
  • 12/15/17 Interviews with Business Office Support Provider (if needed)
  • 1/02/18 Board of Trustees meeting – contract awarded
  • 1/05/18 Contract commencement

IN WITNESS WHEREOF, the parties have executed this Addendum to Service Provider Agreement on the date and year first written above.
