Care Coordination: Acute Hospital Becoming a Preferred Provider in the Narrow Network

There are six programs in the Affordable Care Act that incentivize hospitals to coordinate care with post-acute providers. Improved communication between levels of care, as well as data sharing are just a few of the basic expectations in coordinating care. In this program, participants will learn how health systems are coordinating care with post-acute providers and private duty caretakers through innovation and utilizing new technologies. Participants will hear from thought-leaders in care coordination, readmission prevention and population management. Specific focus will be given to the financial impact of effective care coordination on ACO’s, Bundled Payment Programs (BPIP) and Medicare Spending Per Beneficiary (MSPB).

Effective patient throughput is a challenge for most organizations even though a sound program can significantly affect operational costs, servicecapacity, and the patient experience. Creating good patient throughputincludes strategies that closely align organizational structures, processesand people to initiate discharge execution at the time of admission. Bringing these complex systems together requires an ongoingtrans-disciplinary approach to plan, manage and monitor each area’s metricssuch as length of stay, patient holding and emergency department leftwithout being seen, and patient satisfaction. Results of a sound programmay include increased bed capacity, improved revenue and reduced costs. This discussion will explore strategies to reduce excess inpatient days andcreate bed capacity through multiple interlocking clinical initiatives.
