Academic Suspension Appeal

Instructions

Version: 5/12

Use this form to petition for waiver of academic suspension or readmission to OCU after academic suspension.

In this appeal you are answering an important question:
Why should the Academic Policies Committee allow me to enroll next semester?
How should I answer the questions?
1.  Show us you understand what went wrong – why you have been suspended
2.  Show us you have a plan for success
3.  Show us a change in your attitudes
4.  Provide evidence – actions you have already taken; what you have already done differently
Be thorough. Finish the entire form. The effort you put into this appeal probably reflects how much you really care about turning your academics around.

How do I turn in the appeal?

Submit your completed appeal to the Registrar’s Office in one of these ways:

·  In person or by mail: Registrar's Office, OCU, 1476 Lancaster Pike, Circleville OH 43113

·  By fax: Toll Free: 877.883.9910, or Local: 740.420.5920

·  By email: scan the forms and email them to .

What happens next?

The Registrar’s Office will contact you to make an appointment on the day the Committee is meeting (see your Suspension Letter for that date). If you cannot come that day, we will ask to set up a phone appointment with you and the committee. In-person interviews are generally more likely to be approved.

What happens after the committee has met?

·  If your appeal is granted, you can return on Academic Probation

o  You will register for PS097 Academic Success to help with course planning and homework

o  Your hours will be limited to 12, plus the 1-hour PS097 class

o  You will repeat as many courses as possible in which you earned D or F grades

·  If your appeal is granted, it’s essential that you raise your GPA to an acceptable level the semester you return. A second academic suspension is usually final.

·  If your appeal is not granted, you must sit out a full academic semester (not counting summer sessions). To be readmitted, you will appeal to this committee for reinstatement, and if approved, complete the Former Student Application available from the Admissions Website. It’s extremely rare that a second appeal is granted.
