Service consent forms
Locate your agency's client consent form. Add this document to your Logbook and Resource folder.
Statement of client rights/responsibilities
Locate the document or brochure that your agency issues to clients outlining their rights and responsibilities. This information is likely to be found in the standard information that is issued to clients when they first begin working with your service. If you have difficulty locating this information, see your supervisor for assistance. Add this document to your Logbook and Resource folder.
Confidentiality policy
Locate your agency's confidentiality policy. Add this document to your Logbook and Resource folder.
Client rights in your agency
What processes does your agency use to inform clients of their rights?
Can you describe a situation where you or a work colleague have had to advocate for a client or have had to help a child advocate on their own behalfto ensure their rights were met? Was the process successful? Why or why not?
Record the answers in your Logbook.
Client referral
Locate your agency's client referral procedure/s. If possible, locate a list of other services that your agency regularly refers clients to. This should be a fairly concise list, i.e. a few pages listing service name, contact details and a brief description of the service provided. Do not attempt to include a large folder of service information.
Add these documents to your Logbook and Resource folder.
Logbook Activity -Advocacy services
- For your client group, which advocacy services have you found to be the most useful in assisting your clients?
- For each service, provide its name, contact details and an overview of the services provided.
- Identify a community legal service or centre in your local area which can assist marginalised or disadvantaged clients with legal issues.
- Describe the range of issues for which legal advice is provided, as well as any eligibility criteria that apply.
- Table your findings in your Logbook and Resource folder. A sample table is given below.
