6. BRIEF RESUME OF THE STUDY

6.1 NEED FOR THE STUDY

Greeva Hundanam is a condition in which vitiated Vata lodges in the neck region and leads to Shosha of muscles of the neck.[1] This disease disturbs the day to day activities of an individual and makes him dependent and depressed.

Greeva Hundanam can be compared to Cervical Spondylosis. Cervical Spondylosis is a degenerative condition of cervical spine. The disease is having a prevalence of 0.1-1% in the general population, with a male to female ratio of 3:1.[2] The disease occurs in the middle and later decades of life, during the 4th and 5th decades usually where Vata influence is more in association of Dhatu ksheenata i,e; debility of the tissue built.

Bahya and Abhyantara Snehana treatment plays an important role in the management of Dhatu Kshayaja Vikaras.[3] Therefore, commonly Nasya and Greeva Basti are employed in the treatment of Greeva Hundanam. The same drug administered in different routes shows different action.

Hence there is a need for a comparative study of the effect of Nasya Karma and Greeva Basti with Ksheera Bala Taila in Greeva Hundanam w.s.r. to Cervical Spondylosis.

6.2 REVIEW OF LITERATURE

Greeva Hundanam, as the name itself suggests, is a clinical condition occurring in the neck region.It is the disorder caused when vata is vitiated.[4]

Greeva Hundanam is comprised of two words viz. Greeva and Hundanam. “Greeva” means the back part of the neck or tendon of the trapezium muscle.[5] “Hundanam”is derived from the root “Hudi Samghate”[6] or “Hudi Majjane”,which means to dive,sink, collect, or to be submerged.[7]

Chakrapani while explaning the word Hundanam has given it two meanings.The first one is “Shiro Prabhrutinam Antah Pravesha”.It means inward intrusion of the head and its allied parts,which is possible due to implication with cervical parts.This condition is suggestive of the structural deformity.The other meaning is “Greeva Stambha”,which denotes the restriction of the movement of the neck.[8]

Greeva Hundanamis one among the Vataja Vyadhis in which Vata gets localized in the Neck region and produces constriction of Siras and stiffening of Maamsa Dhatu which further restricts the free movememt of neck.[9]

Common line of treatment of Vata Vyadhis also says about Snehana nasya which includes Brumhana Nasya.[10]

As per Charaka Samhita, Nasya is one of the main treatment modalities when Vata is lodged in between Head and Shoulders.[11]

Ksheerabala Taila is very effective in all 80 types of Vata vitiated conditions. It can be used as Nasya, Abhyanaga, Pana, and Basti.[12]

It acts as Rasayana, Indriyaprasadana, Jeevana, and Brumhana.[13]It contains Bala Moola, Ksheera and Tila Taila. Bala Moola (Sida cordifolia) is having the qualities like Snigdha, Pichchila and is Vata Shamaka and indicated in Vata Vyadhis.[14] Ksheera is Brumhaniya and Vata Pitta Shamaka.[15] Tila Taila (Sesamum indicum) is considered to be the best among oils.[16]

The benefits of Nasya Karma are that the skin, shoulders, face, neck and chest becomes firm and well developed. All the sense organs also become strong.[17]

Nasya is the prime treatment in Urdhwa jatrugata Vikaras.[18]

The Heena Matra of Brumhana Nasya is 16 Bindus, Madhyama Matra is 32 Bindus and Uttama Matra is 64 Bindus.[19]

Vagbhata has described 4 types of oil application on head under Murdhni Taila, which are Shiro Abhyanga, Shiro Seka, Shiro Pichu and Shiro Basti. Greeva Basti is evolved from Shiro Basti procedure. It is a Bahya Snehana and Swedana therapy and more over it is a Sthanika Shamana Chikitsa.[20]

6.3 OBJECTIVES OF STUDY:

1. To study the effect of Nasya karma with Ksheera Bala Taila in the management of Greeva Hundanam.

2. To study the effect of Greeva Basti with Ksheera Bala Taila in the management of Greeva Hundanam.

3. To compare the efficacy of the above two groups to ascertain that which treatment modality is better in providing relief to the patients of Greeva Hundanam.

7. MATERIALS AND METHODS

7.1 SOURCE OF DATA

30 patients of Greeva Hundanam will be selected from OPD and IPD of S.D.M. College of Ayurveda and Hospital, Hassan.

Plane Ksheera Bala Taila will be used for Nasya Karma and Greeva Basti procedure.

7.2 METHODS OF COLLECTION OF DATA

Patients who are fulfilling the criteria for diagnosis and inclusion will be included in this study irrespective of sex, caste, religion.

(a)DIAGNOSTIC CRITERIA

  • Emaciation of Neck muscles and Neck region.
  • Restriction in the movement of Neck.
  • Difficulty in lifting / turning the neck.
  • Confirmed by X ray.

(b)INCLUSION CRITERIA

Patients with Greeva Hundanam (Cervical spondylosis) of either sex and age group of 20-70 years will be taken for the study.

(c)EXCLUSION CRITERIA

  • Patients unfit for Nasya Karma & Greeva Basti.
  • Patients suffering from Amavata.
  • Patients having any infective conditions or systemic disorders.

(d)PLAN OF TREATMENT

Selected 30 patients of Greeva Hundanam(Cervical Spondylosis) will be randomly divided into the following two groups:

Nasya Group:Patients of this group will be administered Nasya as per method given below with Ksheera Bala Taila once daily in the dosage of 8 bindus in the morning (7am-9am) on empty stomach for 7 days.

GreevaGroup:Patients of this group will be treated with Greeva Basti of Ksheera Bala Taila in the morning (7am - 9am) on empty stomach. The duration of the Basti will be 30 minutes.

Method of Nasya Karma

Purva Karma – Mukhabhyanga with Ksheerabala Taila, and Nadi Sveda will be done for 5 minutes.

Pradhana Karma – Patient will be made to lie in supine position, in a room devoid of breeze. The legs will be slightly raised and head slightly tilted upwards, then Ksheera Bala Taila slightly warmed with the help of hot water will be taken in Gokarna and 8 BinduTaila will be instilled into each nostril alternately keeping the other closed. After instilling the drops into the nostrils, the soles, neck, palms, ears etc. will be gently massaged and then the patient will be asked to turn to a side and spit out the phlegm. Then the patient will be asked to lie with face upwards for 2 minutes and wash the mouth with lukewarm salt water.

Paschat Karma: After Nasya, Snehika Dhumapana with Haridra will be given to the
patients with the help of Dhuma Yantra.

Method of Greeva Basti:

Purva and Pradhana Karma:The patient will be asked to lie comfortably in prone position with neck and head straight and well exposed, and arms keeping under head. Then the dough is pasted in a circular manner on this neck. The oil which is pre warmed up to 38- 40 degree C will be poured in the area and filled to the depth of about 3 centimeter. On cooling, the oil will be reheated and replaced at regular intervals so that the temperature is maintained uniform throughout the procedure. This process will be carried out for 30 minutes, once in a day, every morning for 7 days.

Paschat Karma:The oil will be drained out by making a hole in the dough. The dough is then removed and the area will be cleaned with a clean cloth.

Duration for Both the Procedures – 7 consecutive days.

(e)ASSESSMENT CRITERIA:

The assessment will be based on the effect of the therapy on the signs and symptoms of the disease, which will be assigned/graded by putting suitable scores.

Samyak Lakshanas of Nasya as well as any complications will be noted.

Assessment of the patient will be done on the 1st day (Before treatment) and on the 7th day (After treatment), after the procedure to know the result of the therapy.

Assessment of results:

Statistical evaluation of the research will be done by using:

Paired and Unpaired ‘t’ test.

(f)FOLLOW UP STUDY:

After completion of the course of treatment the patient will be followed up for two visits i.e., first after 15 days and second after 30 days.
