Congratulations and welcome to UAB! The Graduate Medical Education (GME) Office would like to help you make your transition to UAB go as smoothly as possible. Much of the orientation process will occur electronically. We utilize an on-line application process called Cactus AppCentral as well as the GME website where you can download all of the necessary forms. Most documents and forms will be returned electronically via Cactus AppCentral or mailed back to the GME Office. We must receive your information by May 15, 2015 to process all required documents so that you are able to start your training on time.

We access MedHub, the GME enterprise software system, for delivery and execution of the Initial Resident Agreement and Code of Conduct. You will receive an email from MedHub Notification when the Agreement and Code of Conduct is ready for your review in MedHub.

We’ve enclosed instructions and a checklist with this email to ensure that you upload all necessary documents and forms. It is very important that your file is complete before you start orientation. You will also receive a packet from us in the mail that will contain an envelope that you can return documents to us.

Orientation for new residents will be held Thursday, June 18, 2015 in the Margaret Cameron Spain Auditorium (maps on GME website). Registration and a buffet breakfast will begin at 7:00 a.m., and you will be able to pick-up your lab coats, scrub suits and ID badge that day. We will start promptly at 7:30 a.m. and run until approximately 4:00 p.m. Please make arrangements to stay throughout the entire day. Department orientation varies, so please check with your program coordinator for department orientation details.

Please bring the following information with you to orientation: Social Security number, visa documents (if applicable), local address and telephone number, Alabama medical license number and Federal DEA registration number (if applicable), Alabama driver’s license number and tag number(s) for your automobiles(s). This information will be needed to complete various forms during orientation.

Form I-9 must be completed before you start your training. This is a two-step process. The first part is completed electronically prior to attending orientation and the second part for most will be completed at orientation on June 18th. It is imperative that you bring one of the original acceptable documents from List A or one document each from Lists B and C (list on GME website) to orientation. If you are using a US Passport, Permanent Resident Card or Employment Authorization Card, bring both the original document and a clear photocopy of the card to orientation.

You will receive your assigned time to attend BCLS/ACLS training (if applicable) and other updates via email.

You will receive an invitation to our electronic online application system on email from When you receive the invitation, you will set up an AppCentral ID and Password the first time you log on. Please set your User ID the same as your email address. In the initial message, there is a statement that says “Please click here to access your initial application process.” Click “click here” and Sign Up for AppCentral and create an account. The form is interactive and used by multiple departments at UAB. The entity your application is in is “UAB Hospital/University of Alabama School Of Medicine you will see an “X” in the box. Detailed instructions will follow.

You will be sent an initial email from MedHub Notification with your username and temporary password. Please select the link in the email to reset your password and login to MedHub. The temporary password will remain active for 24 hours, so please reset your password as soon as possible. Your username will be your first and last initial followed by your 8 digit birth date. You will initially use the site to review and sign the Initial Resident Agreement. You will receive a separate email from MedHub Notification when the Agreement is available for you review. You may access MedHub at https://uab.medhub.com

If you are relocating to Birmingham and will not arrive by June 18th, we have a make-up orientation on July 1st.
