Vermont Mandatory Reporting Requirements Regarding Children
Who Must Report? / ·  Physician, surgeon, osteopath, chiropractor, physician’s assistant, resident physician, or intern
·  Hospital administrator
·  Registered nurse or licensed practical nurse
·  Medical examiner
·  Dentist
·  Psychologist, mental health professional or social worker
·  Pharmacist
·  Any other health care provider
·  School superintendent, school teacher, school librarian, day care worker, school principal or school guidance counselor
·  Camp owner, camp administrator or camp counselor
·  Probation officer or police officer
·  Member of the clergy
Standard of Knowledge / Reasonable cause to believe that a child has been abused or neglected
Definition of Applicable Victim / Child under the age of 18
Reports Made To / Vermont Family Services Emergency Services Program (1-800-649-5285)
Contents of Report / ·  Name and address of the reporter
·  Name and address of the child
·  Name and address of the parents or other persons responsible for the child’s care, if known
·  Age of the child
·  Nature and extent of the child’s injuries together with any evidence of previous abuse and neglect of the child or the child’s siblings
·  Any other information that might be helpful in establishing the cause of the injuries or reasons for the neglect as well as in protecting the child and assisting the family
Timing/Other Procedures / Report may be made orally or in writing. If the report is made orally, a follow-up written report may be requested. Reports to be made within 24 hours.
