Self Care Plan

The purpose of the Self Care Plan is to affirm your personal, spiritual, emotional, etc. needs. The plan itself is personal and should be treated as a method to review your personal growth. This document should be considered a private document that should only be shared if by sharing this information does not alter what you actually put on this form.

Personal Statement: I, Pat McKiernan plan to live life in the most positive manner possible by being a giver who has balance and through this behavior attracts and inspires other to do the same.

Because I am know myself better than anyone else these the things I need to do to maintain quality physical, mental, emotional, social and spiritual health:

1. Treat my alcoholism by attending Saturday Home Group, talking to my sponsor once a week, and living my life according to the 12 steps, which includes daily spiritual exercises.

2. Increase my knowledge and connection to God by going to church each Sunday with my family.

3. Cherish my relationship with my wife by spending quality time with her at least 20 minutes a day.

4. Cherish my children by spending at least 20 minutes a day doing something they want to do and making them feel important during this time.

5. Maintain good physical health by exercising rigorously at least 30 minutes daily.

6. Maintain healthy social connections with friends through some type of recreation once a week.

7. Maintain healthy family connections by talking at least once a week with my siblings and parents.

8. Maintaining a continuous learning cycle by reading for recreation at least one hour per week.

I commit to review this plan at least one time per month to consider changes as my needs change. I will also put this plan in a place where my loved ones can look at it and remind me if I am failing to take care of myself.
