Television and Film Production Nomination Form

Join SAMHSA’s Voice Awards program in recognizing television and film professionals whose work educates the public about behavioral health and demonstrates that people with mental and/or substance use disorders can and do recover and lead meaningful lives.

If you know of a television or film production that aired in a public setting after April 15, 2014, which contains a positive behavioral health storyline, please nominate it for a 2015 Voice Award.

Nominations are open to anyone and there is no limit to the number of nominations an individual can submit.
