a. Student (Indicate what unusual circumstances led to not finishing on time and requested terms/conditions of the appeal.)
b. Major Professor (Clearly explain how the student has remained current in the content of all course work that will expire by the end of the time extension period. If the student has not stayed current in the content, list the specific courses that need to be repeated)
3. Official or Unofficial transcript printed in the same semester as this appeal is filed.
4. Degree plan with all course changes and all course grades and dates included. Include any change in plan forms submitted.
5. Program Coordinator signature below if they endorse the plan put forward in 2a and 2b.
6. Department Chair signature below if they endorse the plan put forward in 2a and 2b and 2c.
All documentation should be assembled by the student and delivered at one time to the Associate Dean for Academic Affairs, Matthews Hall 214 . Students are responsible for delivery of materials.

The normal time to complete a master’s degree is 1.5-2 years for full-time students and 3-6 years for part-time students. Seldom and only in very unseal circumstances should a student take 6 years to finish. If you have exceptional circumstances that pushed your completion over 6 years complete the rest of this form. (note some COE masters plans are longer than 36 hours and allow additional years to complete, check the graduate catalog for details)
List all course work that will be more than 6 years old in the semester of the proposed time extension (not the courses that are 6 years old at the time of the appeal).
Specifics of how the student will demonstrate currency and competency in all courses over 6 years old at the end of the time extension period.
Time line of all relevant activities, accomplishments, and graduation with expected dates of completion:
e.g., repeat any expired course work, oral or written qualifying exams, proposal complete, proposal defense, research and data gathering, completion of writing, dissertation defense, and graduation
