MODEL WORKPLACE VIOLENCE THREAT ASSESSMENT QUESTIONNAIRE

PROBLEM: Although workplace violence isn’t a hazard for all employers, once you find out that a worker has actually been threatened with violence, you’re on notice that a hazard has arisen and need to take steps to deal with it. The first step in the process is to evaluate the seriousness of the threat and then decide what steps need to be taken to deal with it.

HOW TOOL HELPS SOLVE THE PROBLEM: The following Model Workplace Violence Threat Assessment Questionnaire is designed to help you or members of your organization’s threat assessment team, if it has one, perform such an assessment.


Model Workplace Violence Risk Assessment Questionnaire

1.  What types of violent incidents, such as assaults, robberies, threats, etc., are most likely to occur?

3.  How often are violent incidents likely to occur (rarely, infrequently, often)?

4.  Which departments, work groups, job positions, etc. are most at risk? (Designate as high, moderate or low risk.

6.  Are there any deficiencies in the physical facility that create a risk of violence, such as blind spots or poor lighting?

7.  Are there any deficiencies in the security system that create a risk of violence, such as lack of security cameras?

8.  Are there any deficiencies in the company’s policies and procedures that create a risk of violence, such as lack of a visitor policy or no training on workplace violence?
