INSTRUCTIONS FOR PATIENTS REFERRED TO

DR. PATRICK LAVIN’S

NEURO-OPHTHALMOLOGY CLINIC

1. Please come to the Vanderbilt Eye Institute at 2311 Pierce Ave.

2. Please arrive 30 minutes before your scheduled appointment time to allow time to park, find the clinic area, and complete paperwork. Vanderbilt is a large campus; leave ample time to find your way around.

3. Please bring your medical records with you, or prior to your visit confirm that your doctor has sent or faxed your records, or a letter to Dr. Lavin: (fax: 615-936 0223).

4. If you have had a CT or MRI scan bring the films, and any other x-rays, with you. Make sure you have the actual films - not just the written reports. The scans will be at the x-ray department or center where they were performed; your doctor will not have the films, only the reports. These films are necessary for a complete evaluation. If you have difficulty obtaining the scans - call (615) 936-2591.

5. Bring all your medications including all eye drops. Alternatively, the prescriptions or a list of your medications will be adequate.

6. If your symptoms relate to double vision, droopy eyelids, or unequal pupils please bring old photographs of yourself - for comparison.

7. To avoid any scheduling conflicts, please Do not schedule other appointments for the same morning/afternoon. If there is a conflict with these instructions please call (615) 936-2591.

8. If you have any questions regarding the above instructions or require any further information about your clinic appointment please call (615) 936-2591).

9. If you cannot keep your appointment please give at least 48 hr. notice by calling the above number. This will help shorten the waiting list and allow another patient to be seen.
