My signature authorizes Walden Ponds Golf Club to charge my credit card or checking account for the appropriate amount each month for all eligible family members who are enrolled practice and play members. I understand that my membership will be billed using my credit card and that cash payment is not permitted for membership in the program.

PROGRAM CONDITIONS:

The individual that registers in the Player Development Program is the person who has the privileges as stated below. To use the facility under the conditions as stated, participant must have Player Development Card and proper identification at all times. Members must check in with Pro Shop Staff prior to using privileges.

1. Complimentary Practice Balls – Anytime.

2. Three Complimentary Golf Clinics in Season.

3. Player Development Program members can play 9 or more holes for $20.00 at the following times:

a. After 2pm from January- March and October- December

b. After 3pm from April- September

4. PDP Members can bring a guest for $5.00 off the rack rate, per person.

5. PDP members can make tee times 8 days in advance.

6. PDP members will receive a 10% discount on fees associated with renting the Mansion facilities. This discount is also good for any food and beverages (outside contractors and alcohol excluded) for any business or social function other than a golf related event.

7. PDP members will receive a 10% discount on all merchandise in the pro shop.

Walden Ponds reserves the right to make changes and/or amendments to the Player Development Program at its sole discretion including but not limited to monthly fees, cart fees, and time restrictions. Special events, tournaments, maintenance, practices, or weather conditions may restrict or prohibit access to the golf course and/or practice areas at any given time. Participation in special events, tournaments and leagues are not part of the Player Development Program. PDP members will be charged regular rates to participate. Range balls and baskets are the property of the golf course and should be used only on the course practice facilities. Members are not permitted to remove range balls or baskets from course property. Walden Ponds may terminate this contract by giving 15 days written notice, with or without cause. All sales are final, and no refunds may be given. This agreement does not provide the assigned program participant with any rights of membership whatsoever. At the end of the membership agreement, which may be cancelled at anytime after a minimum of one full month period, I must turn in all practice and play membership cards to the golf shop, or notify Westen Lengerich by email at, . I must officially cancel before the 1st day of the upcoming month, or my membership fees will continue to be billed until my membership is officially cancelled. After officially cancelling, I will be removed from the membership and its obligations. A $10.00 fee will be charged for lost membership cards, or membership cards not returned within one month of cancellation.

All benefits of the program are available only while an active member. I agree to all terms and conditions of the program,
