Infant/Toddler Individual Learning Plan

Use the following format to write your individualized learning plan. Be sure to have your supervising teacher/caregiver sign prior to implementation. Then complete the self-evaluation. Learning plans must be typed.

Reason for Activity: Provide a brief review of the child’s development and describe one learning goal for this child based on your observations..

Early Learning Standard: Identify one early learning standard and benchmark for the child and the expereince

Materials Needed: List all the materials you will need for the activity.

Procedure: This should list a step by step procedure. Be sure to include how you will meet your objective. Include how you will introduce the activity, presentation and conclusion to the activity. As you think about the introduction to the activity consider if the activity will be initiated by the child or by the adult. How might that affect the presentation?

Role of teacher/caregiver: In this section consider how you will provide support to the child. Are there variations to the activity you might include? What guidance might the child need?

Self- Evaluation

Following implementation of the activity provide an analysis. Were you able to present the activity as planned? Did you meet your objectives? Why or why not? Be sure to give specific examples. How might you build on this activity or plan a variation of this activity to continue to work to meet the objective. What ideas did you get from the child’s response for future interactions with this child? For future learning plans? Was your role appropriate?

Accommodation: Describe one accommodation you could make in this activity for a child with a special need.
