Objective 1: Create a Form Template
· Start Word 2007.· Start a new document, (or open the document or template that you want to base the template on).
· Insert a table with 7 rows and 2 columns.
· In the left column, type the following field names, starting at the top and going to the bottom of the table.
o Company Potluck
o Name
o Department
o Preferred Date
o Food
o Beverage
· Highlight the top two cells of your table. Then click the Layout tab, and click Merge Cells.
· Highlight/select the Company Potluck text. Click the Home tab and in the Paragraph group, click Center to align the text in the center of the row.
· Click the Design tab. Click the More arrow in the Table Styles group to view all of the preset styles. You may either click on one of these to format your table, or click the arrows beside Shading and Borders to create your own format.
· Click Save As, type the name Company Potluck Form, and select Word Template as the file type. Click Save.

Objective 2: Insert Form Fields
· Start Word 2007 and open the Company Potluck Form template file if you don’t already have it open.· Click in the empty table cell beside the Name field (second row, right column).
· On the Developer tab, in the Controls group, click the Legacy Tools button.
· Point to the various tools and note the ScreenTips that display.
· Click Text Form Field. This will create a place where text can be entered in the cell.
o Click in the cell beside the Department field and insert another Text Form Field.
o Click in the cell beside the Food field and insert another Text Form Field.
· Click in the empty table cell beside the Preferred Data field.
· On the Developer tab, in the Controls group, click Date Picker. This inserts a field with a pop-up calendar so the person filling out the form can select a date using the calendar.
· Click in the cell beside the Beverage field.
· On the Developer tab, in the Controls group, click Drop-Down List. This inserts a field with a pop-up calendar so the person filling out the form can select a date using the calendar.
· Click on the drop-down menu to select it, then on the Developer tab, in the Controls group, click Properties.
· In the Content Control Properties box beside Title, type Click drop-down arrow.
· Click the Add button. Type Soda in the Display Name box and click OK. Repeat this step, adding at least 2 more beverage choices. Click the OK button to close the Content Control Properties box.
· Save the file.

Objective 3: Protect the Document
· Start Word 2007 and open the Company Potluck Form template file if you don’t already have it open.· On the Developer tab, click Protect Document.
· From the drop-down menu, click Restrict Formatting and Editing.
· In the Protect Document task pane, under Editing restrictions, click the checkbox beside Allow only this type of editing in this document.
· Click the drop-down arrow and select Filling in forms.
· Click the Yes, Start Enforcing Protection button.
· In the Start Enforcing Protection box, type in a password, then type it in again to confirm. Click the OK button.
· Click the Office button, select Save As, and save the file as type Microsoft Office Document (.docx).
· Close the file.
· To test the form fields, you can open the file and fill it in by pressing the Tab button to move from one field to another.
· Select Save As, give the completed form file a new name, and then close this file. You can open it to see whether the form fields are correctly filled in.
