Donor Egg Treatment Guarantee/ Shared Risk Program

Dear patient,

You have decided or are considering undergoing an infertility treatment using egg donation at our clinic.

During your treatment, eggs of an anonymous donor will be fertilized by the ICSI method using the sperm of your partner (or donor’s sperm). Created embryos will be transferred into your uterus.

During the donor’s preparations and stimulation followed by the egg retrieval, failure can occur ( no eggs are retrieved), causing cancellation of the entire treatment; we would like you to get familiar with the guarantees that we offer before you even start your treatment.

1. If the failure is caused by the donor (she doesn’t administer her medication, gets ill, terminates the stimulation), you will be immediately offered a back up donor, if at all possible, that meets your requirements and treatment will continue without interruptions.

2. If back up donor is not available, we will immediately offer you the earliest possible treatment date.

3. If donor’s eggs don’t fertilize by the ICSI method (and sperm is of good quality*), your next treatment will be offered to you free of charge.

4. If fertilization doesn’t occur because of poor sperm quality, next treatment will not be offered free of charge.

5. We guarantee at least 2 quality embryos* on the 3rd day of cultivation ready for transfer.

6. If only one quality embryo* is available, you will receive a 33% discount.

7. If no quality embryos are available (and sperm is of good quality*) you will not be charged and you will be offered the earliest possible treatment date.

8. The cost of your medication, used for your uterus lining preparation, will not be reimbursed to you, should the treatment not result in embryo transfer.

9. We do not guarantee a successful acceptance and embryo development by the recipient.

*Sperm of good quality is defined by a sperm analysis, carried out at the time of fertilization, showing motility to be at least 2%, morphology minimum of 2% and volume of the collected sperm sample at least 1 million/ml.

Success of a treatment with donor oocytes depends on the quality of created embryos and the preparation of the uterine lining (endometrium). Pregnancy success rate for a single treatment cycle is around 55-65% (positive pregnancy test) and about 45% for clinical pregnancies (ultrasound confirmed heart beat). During three treatment cycles, almost 90% of all patients should get pregnant. In effort to help our patients undergo their treatment repeatedly, we are offering this shared risk program. We are offering the following terms:

1. During the first treatment cycle, patient pays the full cost of the treatment according to the current price list.

2. Second treatment is also paid in full by the patient according to the current price list.

3. During a potential third treatment, the patient pays only a half of the current treatment cost, and she pays the other half only if she gets pregnant and an ultrasound exam confirms a heartbeat.

4. Shared risk program is offered only for one cycle (process explained above)

5. If a woman gets pregnant and delivers a child as a result of her treatment with us, she is not eligible for a shared risk program anymore.

Unfortunately, we are unable, at this time, to extend our treatment guarantee or our shared risk program to patients selecting PGD as a part of their IVF treatment.
