We take this opportunity to make you aware of an upcoming event celebrating the unique role that case management has in the delivery of high quality and cost-effective health care services.
The American Case Management Association, the first case management association specifically designed to address the needs and concerns of hospital and health system case management and transitions of care professionals, is pleased to announce the 16th National Case Management Week to be held October 8-14, 2017.
The theme chosen this year is Case Management: Exceptional People. Exceptional Care.
At (enter your organization’s name), case management staff members work together as a team to perform the following core functions: clinical assessment, care coordination, patient education, counseling, case monitoring and/or clinical pathway management, discharge planning, resource management and patient advocacy.
We have planned at our institution during National Case Management Week, our annual Vendor Fair. We cordially invite you to our vendor fair, and hope that you can join our department and guests in their information sharing session. Your participation will help us achieve our goal of providing excellent services to our patients and families.
