Date: Write the date you read the book. This is a small space so

It can be written using numbers, like this:

  • 9-8-06

Title: Write the title of the book you read.

Important Reminder: When writing the title of a book, it must be

capitalized and underlined.

Important Note: If you are reading a chapter book, after writing the title
the first time, every time you read thereafter you should write the page
numbers you read, or the chapter title or number. It will look like one of

these:

  • page 16 – 24 (if you read part of a chapter)

OR

  • Chapter 10 (if you read a whole chapter that didn’t have a title)

OR

  • “The Finish Line” (if you read a whole chapter that had a title).

Did you notice that a chapter title is capitalized but has quotation
marks put around it?

Parent Signature: A signature is when someone signs their name to a paper or document. Let your parent know when you are going to start reading. When you are finished with your reading please ask your parent to sign in the column next to your log entry. This is very important.
