The purpose of this form is to gather information and give constructive feedback to the Clergy-in-Charge on his/her performance over the past 12 months from June 20xx to June 20xx. In order to provide a framework for your comments, you are asked to focus on five aspects of the Clergy-in-Charge’s role. Please answer these questions, giving specific examples whenever possible. There is also an opportunity to note any special accomplishments that may not fall into any of the categories listed. The Clergy-in-Charge will be asked to complete a self-appraisal at the same time.

Once the forms are collected, a small group discussion will take place between the Clergy-in-Charge and vestry representatives, including one warden, to review a summary of the comments submitted. All individual comments will remain confidential. A brief overview of the small group discussion will be shared with the vestry at the next regularly scheduled vestry meeting.
