PASSPORT HELPFUL HINTS

ITEMS NEEDED TO APPLY FOR A PASSPORT

***All forms must be obtained from using the online application, once printed there will be a barcode at the top left corner of the of the application***

DS-11 (dated Feb 2008) typed/printed w/bar code (located at the bottom), 2 photos (see Graphics), Military ID, and Original Birth Certificate w/raised seal—NO COPIES—NO HOSPITAL birth certificates.

Or

DS-82(dated Feb 2008) typed/printed w/bar code (located at the bottom), 2 photos (see Graphics), Military ID, Tourist Passport (most recent and was at least 16 years old) or an Expired Official/Tourist Passport no older then 15 years

Box 6 (Mailing Address) Type in the following mailing address:

Box9 (What other names have you used?)

The applicant should list all the names that he/she has ever used. This could be a previous married name, maiden name, former married name, step-parents’, surname. If a different name has been used, must provide proof of name change (marriage certificate, divorce decree, etc…)

Box10 (Parental Information)

The full birth name of the mother and the full name of the father should be indicated even if deceased. If unknown, please indicate “UNKNOWN”. If exact dates of birth are not known, the applicant should approximate the dates.

Box 17 (Permanent Address)

Indicate your place of residence.

Box18 (Emergency Contact)

Please indicate the name of a family member or friend who is not traveling with you, this must be completed or you will not be issued a passport.

Box 19 (Travel Plans), if you are not sure of your travel plans, you’ll type the following info:

Date of Trip: Enter 60 days from the current date that you are submitting the passport.

Length of Trip: Enter 179 days.

Countries to be Visited: Afghanistan * Do not put Iraq, if you do, you will not get a passport.

Box 21/22(Have you ever been issued a US Passport Book/Card?)

Check either YES or NO

If yes, then the most recent passport must be submitted if available, if not mark appropriate box. If lost, stolen you’ll need to complete a DS-64 (form is self-explanatory). You will also need to include your name, passport number, and issue date on both the DS-11 and DS-64.
