My student ______________________________ has been tested to assess his/her proficiency level in Academic English. The results of these tests indicate that my child is eligible for ELL services. At the present time I do not want my child to receive ELL services. I understand that my student ________________________ will not have any services available to him/her at __________________________ School.
Should I decide that my student would benefit from ELL services in the future, I will notify the administrator of the school my student is attending at the time.
