STATE PURCHASING AGREEMENT

FOR

ON-SITE AND OFF-SITE

MOBILE SHREDDING SERVICES 2005-2006 - STATEWIDE

PURPOSE

The purpose of this eQuote is to establish a one (1) year State Purchasing Agreement (SPA), from the date Agreement is approved and accepted by the Department, for the purchase of Mobile Shredding Services by all State of Florida agencies and other governmental entities on an as-needed and called-for basis.

CERTIFICATION

By completing and signing the eQuote, the vendor is agreeing to all terms and conditions on PUR 7722, and certifies that the discounted prices stated will hold for the duration of the ensuing Agreement, if one is awarded. For the Mobile Shredding Service, vendor must use equipment that meets the manufacturer’s warranty, and will assume all responsibility for damages.

TECHNICAL DOCUMENTATION AND PRICE LIST

All services quoted must meet or exceed all requirements of this eQuote. When technical documentation and/or price list is required by this eQuote, its purpose is to demonstrate compliance of the service quoted and to allow an evaluation and verification of a true savings to the State of Florida agencies and other governmental entities.

AWARD

Any Agreement awarded pursuant to this eQuote shall be awarded to the responsive and responsible vendor who conforms in all material respects to this eQuote and offers the highest discount from their list price resulting in the lowest net price to the State of Florida agencies and other governmental entities. Vendor must submit an electronically scanned copy of their price list to Kimberly Hall, the agency contact person, at , or provide a website address for viewing and verifying. The price list from which discounts are taken may not change during the term of the Agreement.

NOTIFICATION/TRANSPORTATION

All prices quoted shall include all charges for arrival, visits, retrieval, pick up, collection, and removal to the mobile shredding equipment at any State of Florida agency or other governmental entity location. Agencies shall call for Mobile Shredding Services when container(s) is/are full or close to it for a 48 hour pickup. The Mobile Shredding Service shall be provided within 48 hours of the request for services. Vendor hereby warrants that all services shall be performed in a timely and first-class workmanlike manner.

PURCHASING CARD PROGRAM

The State has implemented a purchasing card program through the Bank of America, using the VISA network. Vendors will receive payment from the purchasing card in the same manner as other VISA purchases. Accordingly, vendor must presently have the ability to accept VISA or contact a Bank of America in your area, and take whatever steps necessary to implement the ability before the start of the Agreement term. The State reserves the right to revise this program in conjunction with implementation of an on-line procurement system.


VISA acceptance is mandatory but is not the exclusive method of payment. Please indicate your ability to accept VISA in the space provided on the Ordering Instructions sheet of the eQuote. The transaction fee referenced in paragraph seven (7) of PUR 7722 is not applicable when payments are made using the State VISA Purchasing Card. When the State VISA Purchasing Card is not used for payment, then the transaction fee will apply.

CANCELLATION

In the event of the establishment of a state term contract for Mobile Shredding Services, this State Purchasing Agreement will be cancelled.

Experience/Certification:

Vendor certifies that it has at least one (1) year minimum of experience in shredding. On the Price Sheet, please provide the following: the years of experience for your company, any certifications, licenses, bonding, insurance, and memberships held. Upon request by any State of Florida agency or other governmental entity, please provide for review your company’s policies and procedures detailing how the material will be handled while in your possession. Your company’s methods must meet all requirements mandated by the Health Insurance Portability & Accountability Act (HIPAA), the Gramm-Leach-Bliley Act (GLBA), also known as the Financial Services Modernization Act, and be in compliance with the State of Florida Records Retention Schedule.

PERSONNEL

The vendor is to provide supervised and uniformed personnel, trained, licensed, and bonded to perform this Mobile Shredding Service. Vendor must have the capability to perform and complete the services in all respects in accordance with this Agreement document.

Mobile Shredding Equipment

Vendor will destroy documents in their mobile shredding equipment. The shredded material will be brought back to their facility to be repulped into recycled paper products, as indicated in their operating procedures. At all times, vendor will keep the State of Florida agency or other governmental entity property free and clear of excess materials and equipment. The mobile shredding equipment should have the capacity to handle the volume of material generated or produced through the course of government business; and must safely and completely destroy all materials whether stapled, clipped, bound, etc. The mobile shredding equipment must have a cross cut or chip type, premium, industrial-grade shredder to render the material into small unreadable pieces. Strip shredding is inefficient and can be easily reconstructed. There must be a certified scale to weigh the material to be shredded.

WITNESSED SHREDDING

If the State of Florida agency or other governmental entity has copyrighted, royalty-based materials, licensed images, or other highly sensitive documents to destroy, they may require a governmental representative to witness the shredding process. Due to the possibility of proprietary information being lost, leaked, or negligence, a witness must be present in view of the fact that the obligatory responsibility to maintain the confidentiality of the materials remains with the State of Florida agency or other governmental entity.

CERTIFICATE OF DESTRUCTION

The vendor must provide the State of Florida agency or other governmental entity a certificate of confidential document destruction each time this Mobile Shredding Service is performed and invoiced.

DISPOSAL RECEPTACLE(S)

Vendor must provide suitable, secure, covered receptacles and bags at no charge for the term of the Agreement. The receptacles will be returned to the vendor at the expiration of the Agreement. The size and number of the receptacles required will be determined by the vendor and the State of Florida agency or other governmental entity. The State of Florida agency or other governmental entity must discuss the tipping fee with the vendor according to the number of containers needed to service their location and to meet the vendor’s minimum container(s) requirement.

SCHEDULING

The State of Florida agency or other governmental entity is to contact the vendor for prescheduling of Mobile Shredding Services at least 48 hours prior to. The State of Florida agency or other governmental entity is to provide a list with contact information of all of their facilities needing this service, and a central location for the containers to be serviced. The State of Florida agency or other governmental entity is to ensure that the material being shredded is in compliance with the State of Florida Records Retention Schedule. If there are any issues to discuss, the vendor is required to follow-up with the State of Florida agency or other governmental entity. Vendor is to discuss with the State of Florida agency or other governmental entity the requirement to separate the paper from other material that would cause contamination.
