Position Summary: The Summit Community Care Clinic (SCCC) Immunization Coordinator will promote, maintain, and improve individual and community health by supporting patients in adopting healthy behaviors.

The Immunization Coordinator will provide safe, effective, and quality primary, preventive, and evidence-based health care to patients in a culturally competent manner, focusing on health promotion and disease prevention. The Immunization Coordinator provides quality, cost effective, and compassionate medical care and treatment of the defined patient population, with care being as comprehensive as possible within the limitations of SCCC. The Immunization Coordinator will coordinate with Summit County Public health, SBHC CA Immunization Champions, and Lead Clinic Assistant for reconciliations, orders, and other items as needed related to the State Immunization program.The Immunization Coordinator performs related duties as assigned by the supervisor.

The Immunization Coordinator must demonstrate regard for the rights of all patients and provides respectful and considerate care focused on the patient's individual needs. The Lead SBHC Clinical Assistant, LPN, or RN will provide a safe and confidential environment for patients, families and personnel.

Essential Functions:

  • Implements and Coordinates all aspects of SCCC’s Immunization program including compliance with Colorado Department of Public Health and Environment’s requirements. This includes receiving, storing, organizing, monitoring temperatures, and ordering and administering vaccines, as well as ongoing continuing education for clinic assistants and medical assistants regarding immunizations.
  • Provides clinical support to clinic assistants, health educators, front desk staff, nursing, and providers.
  • Consistently, accurately and objectively documents patient interactions and other information in patient electronic health record. Completes vital signs, notes allergies and completes all other pertinent paperwork. Charts lab results, sends reminder notices and other notes. Charting is accurate, timely, and appropriate.
  • Focuses on quality customer service for all patients, coworkers, volunteers and partners outside SCCC.
  • Participates in the Clinic’s QI/QA Program as directed by supervisor. This may include: collection, reporting and/or review of quality data and participation in performance improvement projects.
  • Provides clinical and non-clinical in-services for SCCC staff.
  • Assists medical providers with patient procedures and testing as needed as appropriate to CA, LPN, or RN scope of practice.
  • Assists Clinic Nurse Manager and Chief Medical Officer with projects as assigned.
  • Maintains compliance with Occupational Safety and Health Administration regulations. Responsible for maintaining knowledge of such regulations and for the application of such guidelines in the workplace.
  • Maintains all aspects of patient confidentiality according to HIPAA requirements.
  • Refers and consults with other professionals as needed.
  • Educates patients and family members about health care issues.
  • Attends Staff Meetings and/or reads the minutes.

Additional Tasks/Responsibilities:

These may change and additional duties may be assigned.

  • Thoroughly and accurately document each patient visit and contact in Aprima.
  • Works constructively with Clinic staff to ensure smooth operation of the medical program.
  • Performs other related job duties as assigned by the Medical Director.

Education/Experience: Qualified CA, MA, LPN, or BSN with at least two years’ experience in a clinical setting or its equivalent. Previous non-profit and/or volunteer experience preferred.

Language Ability: Ability to read, write and clearly speak the English language. The ability to interpret a variety of instructions and deal with multiple linguistic cultural variables.

Reasoning and Mental Ability: Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to exercise independent judgment. Self-directing and organized. An ability to reason objectively. An ability to assess, project and plan for patients’ needs. Ability to interpret state/ federal/agency regulations. Ability to document concisely, accurately and in timely manner. Ability to handle a variety of duties which may be interrupted or changed by immediate circumstances.

Interpersonal Skills: Ability to relate cooperatively and constructively with patients, co-workers, administration, physicians and providers, community agencies, referral sources, regulators and other health team members. Ability to enlist the cooperation of others. High tolerance for stress.

Computer Skills: To perform this job successfully, an individual should have a solid knowledge of word processing software, spreadsheet software and database software.

Certificates and Licenses: Current CPR card.

Work Environment:

The work environment is in a clinic which is well-lighted, heated and ventilated where exposure to infections and contagious diseases may occur. The healthcare setting can be fast paced and involve working directly with patients and healthcare professionals. Many of our patients with chronic disease have a minimal level of health literacy and a significant number of barriers to accessing health care. The Immunization Coordinatormay also work in a clerical setting. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions of the job.
