confirm that I authorise the London Borough of Sutton make any enquiry as to my circumstances as they see fit, (including my financial circumstances) and that of anyone else who forms part of my claim including making checks with the Home Office, Police, Health Services, General Practitioners, other Local Authorities or any other organisation or individual who may hold information about me, and I hereby authorise any such organisations or individuals to release to the London Borough of Sutton such information they hold. I specifically authorise my solicitor to provide the London Borough of Sutton with copies of any documents relating to my application/s for leave to remain in the UK on whatever grounds made to the Home Office including but not limited to; any appeals I have made against refusals to grant such leave.

The London Borough of Sutton will collect personal information about me to verify whether I am entitled to support as a person subject to No Recourse to Public Funds.

All personal information collected will be used for the specific purpose it was collected for and will be held securely and protected by the Council. Once we have completed our investigation we will notify you in writing of our decision and the reasons for it.

I confirm that I have today given a full, truthful and accurate account of my history and situation prior to my application to the London Borough of Sutton. If it is determined that your household is eligible for support from the Local Authority and in consideration of such support being provided by the London Borough of Sutton I declare that:

1. I agree to have my photograph taken for the purposes of identification and Home Office checks.

2. I have not concealed or failed to provide any information in response to the questions which have been asked. I understand that failure to disclose all necessary information or to provide false information regarding myself or any of my dependents may lead to information being passed to the Police or other agencies for investigation and to a refusal to provide support or to continue to provide support.

3. I understand that it may be a criminal offence to give false or misleading information or withhold relevant information.

4. I also understand that my application may be rejected and I may be prosecuted if I provide information which is false or misleading.

5. I confirm that I will inform the London Borough of Sutton of any change in my circumstances as soon as it arises, whilst I am being considered for, or are in receipt of, assistance and support from the Council.

6. I will notify the London Borough of Sutton immediately when a decision is made on my application for leave to remain in the UK.

7. If I am granted leave to remain in the UK with access to public funds I will immediately seek accommodation and make an application for all the benefits that I and my family are entitled to and will notify the Council as soon as such benefits are granted. Failure to do so may result in support being suspended and/or recovery action being taken to recover any overpayment.

8. If I obtain limited leave to remain in the UK but with a condition that I have no recourse to public funds that I will take all reasonable steps to have this condition lifted by the Home Office expeditiously. Failure to do so may result in support being suspended.

9. I understand that if I seek to rely at a later date on information I was aware of today, but failed to provide or at a later date, my situation changes in any way and I fail to inform the London Borough of Sutton about my change in circumstances, I may be liable for investigation and/or prosecution as to possible fraud the result of which could mean that I am disqualified from further support.

10. The London Borough of Sutton will provide appropriate support to meet the essential needs of your household, provided that the above conditions are adhered to.

You should complete as much of this section as soon as possible and ask the client to bring documentation to the next meeting - print out List of NRPF Documents and pass to the client. The more information caseworkers have the quicker a decision can be made and support can be offered, if appropriate.

7. Are they an EEA (European Economic Area) national? They may be entitled to some mainstream benefits and able to see employment. / Please read additional advice to support EEA nationals. It is recommended that all EEA nationals presenting to the local authority for assistance are referred for advice in order to establish their entitlement to benefits and/or to check that any refusals of benefits are correct.

9. Do they have any outstanding applications to the Home Office to regularise their stay? / The type of application affects the support that can be offered. In most cases people claiming asylum will receive national asylum support from the Home Office, rather than local authority.
10. Do they have Leave to Remain with NRPF stamped on their visa? / Take a copy of the Visa and upload to MOSAIC.
11. Do they have legal representation, if so what are the Solicitor’s details, and are they happy with the advice they have been getting? / The quality of legal representation makes a big difference to the outcome of cases.
