Physicals

ATTENTION! The UIL has mandated that all 7th, 9th, and 11th graders must have a physical. Amarillo High will offer physicals for the 2014 – 2015 school year on Thursday, May 15, 2014. Physicals will be offered at the AHS Activity Center and cost $15, CASH ONLY . We cannot accept checks. If you choose to get a physical elsewhere, make sure that the current UIL form is used, as a physical on any other form cannot be accepted. There will be a general screening as well as orthopedic screening done by team physicians and general practitioners. Please have the Medical History side of the form completed and signed by both parent and student when you arrive. We will not do a physical without a parent signature. Be sure to use the “Grade” and “School”, for the 2014 - 2015 school year on your forms! Enter the Activity Center by the North door and wear shorts and a t-shirt. Parents are encouraged to accompany the athletes. Below is a schedule of times that groups are to arrive for their physicals.

11th graders (2014-2015school year)

3:00 – Baseball, Tennis, Golf, Girls Soccer, Softball, Boys Basketball, Boys Wrestling, Football

7th & 9thgraders (2014-2015 school year)

4:00 – Crockett

4:30 – Bonham

11th graders (2014-2015 school year)

5:30 –Boys Soccer, Girls Basketball, Volleyball, Swimming, Girls Wrestling, Track, Cross Country, Cheerleaders, Steppers

Be sure to contact the head coach of your sport for specific information. You may also contact the Athletic Trainers (see our info below) for information about Athletic Training Room policies and instructions for when you need our help.

Please print the form below and bring with you to physicals or get a copy of it from your coach.

The link (sports participation packet) below is REQUIRED by all parents and athletes to complete AFTER May 1, 2014. Your athlete can not participate in athletics without the physical/medical history and sports participation packet submitted.
