2016 Registration Form

This is your invitation to join us for the 2016 Put-in-Bay Island Oktoberfest on Saturday and Sunday, October 8 & (, under the big tents in DeRivera Park. Event hours are Saturday from 11 a.m. to 7 p.m., and Sunday noon to 5 p.m.

Invitations for food booths are extended ONLY to Put-in-Bay Chamber of Commerce Full Members in good standing (meaning all Chamber bills are current) that have Food Service Licenses. You must provide a copy of your company’s food service license with this form and your payment when registering for Oktoberfest.

The space being leased is an 8-foot table. You will have to provide your own skirting and table linens and decorations. It is the vendor’s responsibility to protect the table from any damage. Any vendors damaging tables will be accessed a charge to replace the table. You are expected to be set up and ready for business by opening each day. The Festival Committee has reorganized the set up of the tents in order to accommodate both patrons and vendors. We are celebrating our 23nd year and many food vendors have been our Oktoberfest partners since the beginning. With that in mind, our long-time participants will be treated accordingly when booth space and table placement is assigned.

Please complete this form and provide the requested information, including the names of the German foods you plan to sell. As in the past the Chamber will sell beer and wine. Individual vendors are prohibited from selling soft drinks, water, wine, coffee, and any alcohol. Registration forms must be returned to the Chamber office by Friday, September 30th . NO GRILLS OR PETS INSIDE THE TENT. We’ve also enclosed a copy of the Ottawa County Health Department permit application for your convenience. OCHD has contacted the Chamber and is insisting that applications be in 10 days prior to the event.

All food vendors must display a copy of their current food service licenses/permits at their booth.
