2017 CMC3 Foundation Scholarship Instructions for a Faculty Nominator

Thank you for nominating a student from your college for a CMC3 Foundation Scholarship.

Complete application packets must be received by Wednesday, March 15, 2017. All forms and supporting documentation should be sent through the postal mail in one envelope to:

At the time of application, the student applicant must meet the following criteria:

$ Have successfully completed at least 30 college units1, including at least 8 units at a CMC3 member college2.

$ Be currently enrolled in a minimum of 6 units at CMC3 member college2.

$ Have completed at least one mathematics course at the level of 2nd-semester engineering calculus or higher.

At the time of application, the faculty nominator must meet the following criteria:

$ Be a current member of CMC3. If necessary, contact the membership chair www.cmc3.org/board.html
for verification or become a member www.cmc3.org/join.html at least a few weeks before Mar. 15.

$ Be a mathematics faculty member at a CMC3 member college2.

$ Nominate no more than two students for 2017.

In order for a student to be considered, the following must submitted to the address above.

$ A Nomination Form (below) filled out by the nominating faculty CMC3 member.

$ An Application Form filled out by the student. Forms are on www.cmc3.org/foundation.html#scholarships

$ A copy of the student’s college transcripts (unofficial transcripts are acceptable at the time of application,
but official transcripts will be required before an award is made).

$ Two one-page letters of recommendation on official college letterhead from college mathematics faculty,
at least one of whom the student has taken (or is currently taking) at least one transfer-level3 college mathematics course. Letters need not be from CMC3 members, but at least one of them must be from mathematics faculty at the CMC3 member college that the student is now attending.

A faculty nominator may nominate up to 2 students. If your college wishes to nominate up to 2 additional students you will need a second CMC3 member to nominate them. No more than 4 applications will be accepted from any one college.

3 Transfer-level mathematics is Trigonometry or above.
