THE LOUIS STOKES ALLIANCES FOR MINORITY PARTICIPATION (LSAMP)

2016-2017 SOPHOMORE SCHOLAR AGREEMENT

The Louis Stokes Alliances for Minority Participation (LSAMP) is pleased to grant to me, the undersigned student, a stipend of up to $750 for autumn semester2016 and up to $750 for spring semester 2017. In order to receive the stipend at the end of autumn semester 2016 and spring 2017, I must be enrolled in a STEM major and meet the following expectations and participate in the following activities each semester:

  1. enroll in and complete a minimum of 12 credit hours (full-time) at The Ohio State University Columbus campus;
  2. maintain a minimum cumulative and semester grade point average of 2.8 or above;
  3. understand that the stipend payment will be made payable to me and the University, per the University Bursar (If there is a balance due, that balance will be paid with the LSAMP funds.);
  4. agree that stipend funds will be used to defray educational costs such as tuition, fees, books, and supplies;
  5. agree to forfeit and return to LSAMP the full amount of the stipend within thirty (30) days of change in status from full-time to part-time student, or withdrawal or dismissal from The Ohio State University;
  6. understand that the LSAMP award is issued to me via Student Financial Aid. By federal law, no student can be awarded aid in an amount that exceeds the estimated cost to attend The Ohio State University as established by Student Financial Aid. Therefore, if I am receiving a complete aid package (tuition, fees, room, board, and miscellaneous expenses) through federal grants and loans, the LSAMP award will substitute for all or a portion of that aid with no net increase of fund to me;
  7. agree to furnish to LSAMP such records, reports, transcripts, and certificates as LSAMP may reasonably request. I also agree that LSAMP may contact The Ohio State University Registrar to obtain such information as LSAMP may deem appropriate in connection with the stipend;
  8. agree that LSAMP may contact me for purposes of promoting its program, and agree, if contacted, to voluntarily provide and let LSAMP use photographs, quotations, or other information to help LSAMP in its public relations and fundraising efforts;
  9. participate in at least 4 success coaching sessions with the LSAMP Program Coordinator each semester; (Additional coaching sessions may be requested or required based on the needs of the individual LSAMP Scholar.)
  10. participate in at least 16 academic resource sessions (office hours, ODI tutoring or supplemental instruction, study group sessions, private tutoring, math/stats lab) appropriate to my academic program per semester;
  11. meet with my peer mentor at least 4 times per semester;
  12. meet with my faculty mentor at least 2 times per semester;
  13. attend at least 4professional development workshops approved by the LSAMP Program Coordinator;
  14. complete any assessment surveys needed by LSAMP per semester;
  15. participate in under graduate research
