I would like to volunteer my services for the 2013 Winterfest Celebration and agree to the following:

1.) To fulfill all confirmed shifts and arrive on time for each shift.

2.) To wear the official volunteer t-shirt during my shift(s). If applicable.

3.) Not to be under the influence of alcohol or drugs during schedule shifts. (If you wish you consume alcohol after your shift, please remove volunteer t-shirt or other Winterfest material (such as banners on boats) and change into your regular shirt.)

4.) To be an ambassador of good will and help the festival patrons in any and all capacities.

5.) All shifts will be filled on a first come, first serve basis. I understand that I may be asked to fill in another area if the one I am requesting is full.

I, the undersigned, do hereby forever discharge, release and hold harmless Winterfest, Inc. and its sponsors, of and from any and all manner of action, suits, damages, or claims whatsoever arising from any loss or damage to the person or persons or property of the undersigned while in the possession or under the supervision of Winterfest, Inc. I do hereby consent to all rules and regulations established for the event and understand that the Event Coordinator will have full authority. I understand that failure to abide by the above stated guidelines will result in my immediate dismissal of the event.

RELEASE OF LIABILITY AND PERMISSION REGARDING PICTURES:

Screening process - You may be required to complete a form that will allow the Seminole Hard Rock Winterfest Boat Parade to perform

a police records verification on your name and update such verification. You may also be asked to submit fingerprinting. Please bring a

photo ID when you come to the Volunteer Office for your interview. Please note that teenagers 16 and 17 years of age are also subject to

this procedure.

Undertaking to act in a responsible and safe manner - I hereby undertake and agree to act in a responsible and safe manner at all times

while acting as a Winterfest, Inc. volunteer.

Insurance - Winterfest, Inc. maintains insurance to the benefit of its volunteers while they are acting as such. This insurance coverage is

for claims arising out of injuries to third parties which are caused by the negligence of the volunteer and for accidental death or

dismemberment of a volunteer that occurs

Permission to use pictures - I hereby grant to Winterfest, Inc. and persons acting on its behalf the unrestricted and perpetual right and

permission, in respect to pictures or film footage that it, through its photographers and/or contractors, has taken of me or in which I may be

included with others, to use, to publish, to broadcast, and to authorize a third party to use, publish or broadcast the same in whole or part, in

any and all media now or hereafter know, for the purpose of promoting volunteerism and/or Winterfest, Inc.
