August 16, 2011

TheAugust meeting of the Cutchogue Fire District was called to order by Commissioner Chairman Joseph S. Zuhoski Jr., at the Cutchogue Firehouse at 8 pm with the Flag Salute.

Roll call was taken as follows:

Minutes of the previous meeting have been printed and distributed to each commissioner. Treasurer Peter Zwerlein financial report was submitted, by the secretary, for board approval.

Motion by commissioners Sanford Friemann and Robert Fisher to approve the minutes and financial report, motion carried unanimously.

Treasurer Zwerlein submitted bills for the period totaling $82,755.52

Motion by Commr Brewer, seconded by Commr Fogarty to approve the bills, motion carried unanimously.

Correspondence:

A letter from Chief Fohrkolb placing member Loretta Garbarino (509) on the medical inactive list as of August 1, 2011.

Letter received from United Fire Company No #1 informing that John Hinton was elected new member #550 at the August 4th meeting. Also that member #540, Brian Tyler’s, resignation was accepted followed by a motion from Commr Fogarty, seconded by Commr Zuhoski to approve John Hinton as a new member and the resignation of Brian Tyler, motion carried unanimously.

Committee reports:

Commissioner Friemann advised all present that we should make changes in our present LOSAP system annuity payments thereby saving the district money. We are now purchasing an annuity for retiring members in one lump payment. It is recommended that we amend the program to make monthly payments directly from the fund in effort to reduce the yearly expense. After a brief discussion, Commr Friemann moved that we adopt the new payment plan commencing with two members now retiring. Motion was seconded by Commr. Zuhoski and carried unanimously.

Commr. Friemann went on to inform the members that David Fohrkolb has donated a small bucket truck to the Fire District and that the district will provide a tax deduction for the gift. Commr Friemann proposed the following resolution attached further in these minutes.

August 16, 2011 continued:

Commissioner Brewer announced that the original electric key locking system for the firehouse has encountered major component failure and requires upgrading. The present system is now obsolete and other sources have been looked into. The Millennium System is on NY State Bid Contract and available to us as a fire district. Discussion followed about the addition of more doors to the lock system and to add the “Door Ajar” feature to each entry door. Also considered is the barn entry door. The new system can easily handle this type of installation and to include the wiring of the buildings.

Our fuel pump system was also mentioned. Motion by Commr Brewer and seconded by Commr Fisher that we repair and upgrade the building lock system as budget allows for this year, motion carried unanimously.

Commissioner Fisher no report at this time.

Commissioner Fogartystated that he has hired Kyle Pumillo as a firehouse attendant as of July 2011.

Commissioner Zuhoski discussed the upcoming budget preparation and asked the Chief for his recommendations. The preliminary budget is due at the Town by Sept. 27th. The board agreed upon the date of August 31st at 7:00 PM to meet with the chiefs to review their requests.

Chairman Zuhoski also mentioned that the present 8-5-23 car is being decommissioned and to be replace by the 8-5-32 car.

Chief Fohrkolb’s Report:

Activities:

  1. New York State parade and drill in Central Islip, August 20th & 21st .

10:30 am Fire Police will be needed at CI.

  1. Annual chicken barbeque, Saturday, August 27th. FIRE & EMS Alarm coverage during the barbeque will be provided by the Mattituck, Southold, Greenport & Eastport fire departments. Two ambulances, one engine and one heavy rescue truck will be located at Cutchogue fire headquarters from 1530hrs to 2300hrs.
  1. Hagerman Drill, Saturday, September 3rd

Motion by Commr Fisher, seconded by Commr Fogarty to approve the above requests, motion carried unanimously.

August 16, 2011 continued:

Requests:

  1. Request to have grounds sprayed for the chicken barbeque and family picnic.
  1. Purchase ten (10) sets of lion brand turnout gear suspenders to replace worn suspenders at $35.00 each. Total cost: $350.00
  1. Replace two (2) non functional eagle 160 thermal imaging camera batteries at $133.00 each. Total cost: $266.00 (see attached price quote)
  1. Purchase twelve (12) breakaway reflective safety traffic vests at $50 each. Total cost: $600
  1. Removal of all VHF low band portable radios from fire department apparatus. The low band portable radios have not been used in over two years since the high band radio system was implemented. For interoperability with other fire departments, statewide UHF mutual aid channels have been programmed into all CFD high band radios.

Motion by Commr Zuhoski and Commr Fogarty to approve above items 2,3,4, motion carried unanimously.

Repairs:

Engine 8-5-4 light bar rotator motor burned out, 8-5-19 light bar corner strobe and LED module burned out, 8-5-32 Tahoe light bar corner strobe burned out. Engine 8-5-3 light bar bulb burned out. Ambulance 8-5-16 light bar rotator motor burned out.

Install High Band radio from present 8-5-23 in Bus 8-5-9. The district has about 20 old high band portable radios that are of no use to us now and we should consider donating same to a needy department or district. Chairman to take under consideration and to have our mechanic repair all strobe lights.

Rescue Captain Hartmann informed that there is an “End of Run Sheet” now in use for all ambulance calls.

Bucket Truck Acceptance

A Regular Meeting of the Board of Fire Commissioners of the Cutchogue Fire District, Town of Southold, Suffolk County, New York, was held at the Cutchogue Firehouse, 260 New Suffolk Road, Cutchogue, New York, on the 16th day of August, 2011, at 8:00 p.m., prevailing time.

PRESENT: Joseph S. Zuhoski, Jr., Commissioner Sanford Friemann, Commissioner

Arthur Brewer, Commissioner

Robert A. Fisher, Commissioner James R. Fogarty, Jr., Commissioner Peter Zwerlein, Treasurer

Matthew J. Martin, Secretary

ABSENT:None

Commissioner Friemann discussed a proposed gift of a 1997 Ford F477 Bucket Truck, Vehicle Identification #1FDLF47FOVEC20998, presently owned by David M. Fohrkolb,

residing at 840 Manor Hill Lane, Cutchogue, New York 11935, to the Cutchogue Fire District. Commissioner Friemann explained that David M. Fohrkolb had contacted him and desired to make the unrestricted gift of this Bucket Truck for the general use of the Cutchogue Fire District. Commissioner Friemann stated he told Mr. Fohrkolb to put this in writing and he would have the Fire District mechanic, Henry Rienecker, inspect the vehicle. Mr. Rienecker reported after inspection of the vehicle that it was in good mechanical working condition.

August 16, 2011 Continued:

Commissioner Friemann then stated he had James P. Hines of Buzz Chew Commercial Truck perform a vehicle evaluation along with its fair value. Mr.Hines stated after his inspection that the vehicle was in good condition with low mileage and had a fair value of $13,500.00. Commissioner Friemann then stated that he asked the Fire District Attorney, Richard F. Lark, to review the details of the gift and to prepare the necessary paperwork to accept the gift subject to approval of the Fire Commissioners. Commissioner Friemann stated this would be a tax deductible gift on Mr. Fohrkolb's part. Commissioner Friemann presented to the meeting a Declaration of Gift by David M. Fohrkolb dated July 29, 2011 and a copy of James P. Hines evaluation dated 8/02/2011.

NOW, after discussion amongst the Commissioners, on motion of Commissioner Friemann and seconded by Commissioner Brewer, all members present voting therefore, it is

RESOLVED, that the Cutchogue Fire District accept the gift from David M. Fohrkolb of the 1997 Ford F477 Bucket Truck, Vehicle Identification #1FDLF47FOVEC20998, and it is

FURTHER RESOLVED, that the Chairman of the Board of Fire Commissioners be authorized to execute an Acceptance of Gift and deliver the original to David M. Fohrkolb and a duplicate original of the Acceptance of Gift is ordered to be appended to the minutes of this meeting along with the original Declaration of Gift and a copy of the evaluation of James P. Hines, and it is

August 16, 2011 continued:

FURTHER RESOLVED, that the Chairman or the Treasurer of the Fire District be authorized to execute on behalf of the Fire District IRS Form 8283 acknowledging the gift on behalf of the Cutchogue Fire District.

The question of the adoption of the foregoing resolutions were put to a vote on roll call, which resulted as follows:

Martin, Secretary of the Board of Fire Commissioners of the Cutchogue Fire District, Town of Southold, Suffolk County, New York, do hereby certify that I have compared the foregoing resolutions adopted at a regular meeting of the Board of Fire Commissioners of the Cutchogue Fire District on August 16, 2011, with the original thereof on file in my office and that the same is a true and correct transcript therefrom and of the whole of said original resolutions; and that due notice was given to all members of the Board of Fire Commissioners of the Cutchogue Fire District of the foregoing meeting.

DECLARATION OF GIFT

I, DAVID M. FOHRKOLB, residing at 840Manor Hill Lane, Cutchogue, New York 11935, am the owner of a 1997 Ford F477 Bucket Truck, Vehicle Identification #1FDLF47FOVEC20998, which I represent is in good operating condition.

August 16, 2011 continued:

I desire to make an unrestricted gift of this 1997 Ford F477 Bucket Truck to the Cutchogue Fire District for its general use. To carry out my purpose, if this gift is accepted, I hereby give and deliver to the Cutchogue Fire District this

1997 Ford F477 Bucket Truck, Vehicle Identification #1FDLF47FOVEC20998. It is my purpose and intention to vest all incidents of my ownership in the vehicle to the Cutchogue Fire District.

ACCEPTANCE OF GIFT

The Cutchogue Fire District, Town of Southold, County of Suffolk, State of New York, hereby accepts a gift from David M. Fohrkolb, residing at 840 Manor Hill Lane, Cutchogue, New York 11935 of a 1997 Ford F477 Bucket Truck, Vehicle Identification #1FDLF47FOVEC20998, with a value of $13,500.00, as appraised by James P. Hines of Buzz Chew Commercial Truck dated 8/02/2011, which is attached hereto as Exhibit A.

August 16, 2011 continued:

It is agreed and acknowledged that the Cutchogue Fire District is accepting the gift of the 1997 Ford F477 Bucket Truck for the general use of the Cutchogue Fire District.

This acceptance is executed pursuant to a resolution of the Board of Fire Commissioners adopted on August 16, 2011.
