The following tables consist of various fields to create SAPs that will pull the same records that the CDA checks for. For example, the first table below corresponds to Referral Check 5. The last section includes instructions for looking at the MAYSI dates in Excel.

Navigate to the file
Right Click
Open with Excel
Select Column A
Select Text to columns (in 2007 it’s found in the Data tab)
Click Next
Check the following boxes
Comma
Other in the textbox for other enter ‘(‘ this will isolate the PID numbers
Click Next
In the preview window, click the date clomuns and select the date radio Buttons (see screen print)
Click Finish
In cell H2 enter the fiormula =e2-f2
Copy the formula down the entire column until you reach the end of your data
Hit Ctrl + end to go to the bottom of your data
Select the end of your data and highlight everything to cell a2
Click sort
Select Column H, largest to smallest
Scroll down until clomn h value is equal to 13, anything below this is within the required timeframe
