To receive CME credit for a live broadcast, please complete the following steps:

  2. If you have any questions that you would like to ask the speaker during the broadcast, e-mail them to .
  3. Complete the below Attendance Form and Evaluation Form (The evaluation form is completed only twice a year). Please do not put your name on the Evaluation Form. You can print the filled in forms and email or fax them.
  4. Return the completed forms to the department within 1 (one) hour of the conclusion of the live broadcast. Any form returned after that time will not be accepted. The forms may be sent to the department in one of two ways:
