I understand that my consent to these provisions is given in consideration of the acceptance of this application and for being permitted to participate in this event. I recognize and assume the risks of illness and injury inherent in any exercise program and I am participating in the 2006 San Francisco 49ers Anthem Audition upon the express agreement that I am hereby waiving and releasing any and all rights and claims for any damages which I may have against the San Francisco 49ers, National Football League, E2K / Olmstead Productions, Pier 39, as well as any other person connected with the event, their heirs, executors, successors, assigns, administrators, for any and all injuries I may suffer for participating in any parts of the event as a result thereof. I give my full permission to the San Francisco 49ers and e2K / Olmstead Productions and its local affiliates to use any photographs, videotapes, or other recordings of me that are made during any parts of the event.

All Applicants Must Submit a Demo Tape

Please Include a cd, dvd, vhs or cassette tape of you or your group singing the National Anthem. It does not need to be professionally recorded, but please note that the first phase of judging will be based on your demo tape. From there, the top 30 finalists will attend the live audition. Finalists will be notified by July 15th, 2006.
