Instructions – READ CAREFULLY

  1. Both Students and Parent/Guardian must sign the application
  2. This form is for ONE (1) voice part only. Student may audition for Voice and Piano/Organ on separate audition forms. Each application must include a $5.00 audition fee.
  3. Director and Principal must sign the application.
  4. The Director should send all applications and ONE (1) check payable to “PMEA District 10”, covering all application fees to the address below. Please double check all applications for accurate information. Do not send separate checks.

in District 10 events and will not hold PMEA or its members responsible of the school’s performing organization. The signing Director

for unforeseen accident or illness, or loss or damaged instruments or MUST be a member of PMEA and will participate in all auditions
equipment. in order for the student to be eligible for District, Region, and State events.

By signing this form, you acknowledge reading and understanding this item: “Students should not apply to participate in festivals if, for any reason (including religious activities), they plan to miss part of the affair. A student must participate in the complete festival program commencing with registration and concluding with the final concert, except of illness. Any further exceptions must be cleared with the Host and the District President.”
