1.PURPOSE: This procedure describes and explains the Vicor process for requesting and receiving Material Declarations from Vicor Suppliers, and the process for preparing Material Declarations upon request for Vicor Customers.
2.SCOPE: The procedure impacts Vicor Quality personnel, RoHS Program Management and Vicor Supply Chain personnel. Westcor and Picor are mentioned in the following paragraphs for reference only.

4.PROCEDURE
4.1.Throughout the World today, several nations are instituting[Quality Assurance]
Legislation which concerns itself with the monitoring, control and/or elimination of various types of hazardous materials. This Legislation includes EU RoHS, EU WEEE, China RoHS, California RoHS, Korea RoHS, EU REACH, and others in development or being launched.
4.2.A review of the requirements of all of these Directives high-lights the importance of Material Disclosure throughout the Supply Chain, from Vicor Suppliers to Vicor Customers.
4.3.Throughout the implementation of EU RoHS Compliance at Vicor, as outlined in QSP-0029, the request for Material disclosure and the receipt of said disclosure was paramount to achieving compliance.
4.4.Vicor’s initial request for material content of all purchased items was made via e-mail, letter, fax and phone calls. No format for these requests or receipts was established; they were received in various formats which best suited the information provider. This information was transferred into the format of the “Green Refreshed List,” which developed as a single repository location for the myriad various material content communications from Vicor suppliers.
4.5.In like manner, Vicor Customers instituted a series of self-designed material content spreadsheets and data transfer forms to collect, transfer and store material content information of Product purchased from Vicor. These material content formats were developed as a result of there being no central, standardized format for Material Declaration Requests.
4.6.Recognizing the need for a standardized Material Declaration format, the Association Connecting Electronics Industries (IPC) promulgated two Forms in 2005. These Forms, IPC-1752-1 and IPC-1752-2, were last updated in September, 2007, and will be described in the upcoming paragraphs.
4.7.IPC-1752 Forms
4.7.1.IPC-1752-1 and IPC-1752-2 usage are described in Document IPC 1752. This Document was developed to describe, in detail, a Material Declaration Management Program using the two IPC Forms. Vicor has instituted a Material Declaration Program based on the Document and the Forms.
4.7.2.The IPC Forms allow for providing Material Declarations in varying complexity levels based on the Declaration Classification required and the IPC Form utilized. IPC-1752-1 covers Declaration Classification 1 through 4 and IPC-1752-2 covers Declaration Classification 1, 2, 5 and 6. These Declaration Classifications are outlined in Table 1.

4.7.3.Vicor initially determined that Suppliers would be requested to provide Declaration Classification 1 for all parts supplied, using IPC-1752-1. Phase I and Phase II of the Material Declaration effort requested this level of Material Declarations from Vicor Suppliers.
4.7.3.1.With the proliferation of Environmental Legislation, the ever-increasing complexity of Customer Environmental Requirements, and after full understanding of the requirements of EU REACH, it was determined that Full Disclosure, per Form IPC-1752-2, Classification 6, would be required and requested of all Suppliers.
4.7.3.2.Refer to paragraph 4.8 for a complete description of Vicor’s Full Disclosure Material Declaration effort, known as Phase III.
4.7.4.Westcor initially determined that IPC-1752-1/2 would not be utilized. Westcor maintained Supplier Certificate of Compliance data for each part received in accordance with a Westcor drawing, which states the RoHS status. For board level catalog items, an Excel spreadsheet of RoHS Compliance was maintained.
4.7.4.1.The Material Declaration Phase III effort carried out by the RoHS Compliance Manager at Vicor Andover will also cover all purchased components received by Westcor and Picor.
4.7.4.2.Westcor and Picor receipts will be covered by Full Disclosure Material Declarations per IPC-1752-2.
4.7.5.For all Customer Material Declaration requests, Vicor will complete all Customers self-developed Spreadsheets and Material Declarations received. In all cases where the Customer has not instituted a self designed format for Material Declarations, Vicor shall initially complete and submit Material Declaration Classification 4 using IPC-1752-1. After completion of the Material Declaration Management Database for maintenance of Material Content data and the receipt of Full Disclosure Material Declarations for all Vicor parts, Vicor will provide Material Declaration classification 6 on Form IPC-1752-2.
4.7.6.For all Vicor Andover, Westcor and Picor Supplier Material Declaration receipts, the RoHS Compliance Manager shall be the main point of contact. For all Customer Material Declaration requests, Vicor Andover, Westcor and Picor, the RoHS Compliance Manager shall be the main point of contact and the primary response person.
4.8.Supplier Material Declaration Requests
4.8.1At the beginning of Phase III, a Master RoHS Supplier AVL Material Declaration Parts List was run, listing all parts created up to the present. This includes two separate listings, one from Peoplesoft of Andover and Westcor purchased components, and one from Omnify of Picor purchased components.
4.8.2All Material Declaration requests to Vicor Suppliers during Phase III will utilize Form IPC-1752-2. A Master Form shall be created for each Supplier, listing the Requestor Information, plus the individual Suppliers name and the suppliers Vicor assigned Supplier Identification Number. An Instruction Sheet will also be created to be supplied with every transmission to Suppliers. This instruction sheet provides all required information for proper completion of the IPC Forms, and requests that Forms be completed for Full Disclosure of the Material make up of components supplied to Vicor.
4.8.3The RoHS Compliance Manager shall develop a form letter to be delivered to each Supplier requesting the completion of all required IPC Forms. This letter will be tailored for each Supplier and will be delivered via Email to each Active Supplier. Along with this letter, the RoHS Compliance Manager will deliver an appropriately formatted master IPC-1752-2 Form, the instruction sheet and an extract from the Master RoHS Supplier AVL Material Declaration Parts Lists listing only the parts provided by that particular Supplier.
4.8.4All IPC Forms received from Suppliers for Purchased Parts will be processed by the RoHS Compliance Manager.
4.9Material Declaration Processing
4.9.1All Material Declarations per IPC-1752-2 returned to Vicor by Suppliers shall be reviewed for accuracy, content and completeness by the RoHS Compliance Manager.
4.9.2Each form shall be reviewed to ensure:
4.9.2.1The Supplier has included Supplier name, Contact person, Contact Title, Contact Phone and Contact Email.
4.9.2.2Each Form should address a single Vicor Item Number, or a Vicor Parent part Number with a list of all relevant dash numbers provided. Item Nomenclature, Manufacturers Part Number and Nomenclature, Item Revision, Effective Date, Item Weight and Manufacturing Site shall be provided for each Vicor Item Number.
4.9.2.3Each Form returned should indicate the RoHS Status of the part the Form addresses. RoHS Status shall be indicated in the block titled “RoHS Declaration” and will contain codes as listed in Table 2:

4.9.2.4The Supplier shall include either the Simplified or Detailed Legal Statement. Vicor prefers the Simplified Statement.
4.9.2.5The Form shall be electronically signed by the Authorized Representative. It is not acceptable for Suppliers to print the Form, mechanically sign, scan the Form into PDF format and return to Vicor. The database described in paragraph 4.9.6 requires that the Forms be returned electronically, so that the XML Form in the background can be used to export the data into the database.
4.9.3After the Form has been reviewed for accuracy and completeness, it shall be saved in a secure Network location entitled: RoHS_Supplier_Certifications on Beethoven. Each Form shall be saved using a name in the Form: Form IPC-1752-2 Supplier Name Response Document ID Part Number Date Returned.
4.9.4Each Material Declaration receipt shall be data entered into Peoplesoft. Open Purchasing Attributes and access the RoHS Supplier Certification Panel. Enter the Vicor Item Number, the applicable Supplier ID Number, the Certification Type (which matches the Declaration Class on the IPC-1752-1 Form) and the RoHS Certification Number (the name used to save the Certification in the Network location). Check the box RoHS Certification Received.
4.9.5Westcor Material Declarations shall be saved and data entered in a similar manner to Andover documents. Picor Material Declarations shall be saved to the same Network location, but they will not be data entered in Peoplesoft.
4.9.6All Material Declarations received from Suppliers, whether for Andover, Westcor or Picor components, shall be received electronically in PDF Format. Each Form shall be imported via XML Format into the Material Declaration Management Database.
4.10Customer Material Declaration Requests
4.10.1As indicated in Paragraph 4.7.5 above, Vicor will complete all self-designed Material Declaration formats requested by Customers for completion. In like manner, all Customer requests for specific RoHS Certificates of Conformance shall be completed by the RoHS Compliance Manager.
4.10.2In cases where a Customer has not instituted a self-designed Material Declaration format, Form IPC-1752-1 shall be used to furnish Material Declaration requests from Customers, until all Vicor components have been covered by Material Declaration submittals from Suppliers and the Material Declaration Management Database has been completed. A that point, Vicor will supply Form IPC-1752-2, classification 6, in answer to all Customer requests.
4.10.3Individual Customer Forms will be stored in a Network location entitled: customermaterialdeclarations on Beethoven.
