Section 3602-e(13) of Education Law requires each school district operating a prekindergarten program to report annually to the Board of Regents, the Commissioner of Education, parents, teachers and the public on the status of the program. Submission of this report fulfills this requirement.

The 2011-2012 UPK Final Program Report consists of two parts. Part 1 consists of Summary of Student Progress, which is page 2 of this document. Part 2 is an EXCEL database comprised of two worksheets (see worksheet tabs located on the bottom left of the spreadsheet); one entitled “11-12 UPK Sites” and one entitled “11-12 Fiscal Summary.”

Both Part 1 and Part 2 must be completed and then transmitted to the Office of Early Learning as attachments to one e-mail to the below address. When emailing your final report, be sure to include the district name and UPK final report in the subject line.

SUMMARY OF STUDENT PROGRESS

Directions: For each area identified in the below-chart, indicate the assessment tool(s) used to establish a developmental baseline (pre-test) and measure children’s progress (post-test). Then provide an analysis of this assessment data. The analysis should aggregate the data and compare children’s skill levels in the fall with the levels achieved in the spring. It should provide a description of the growth of your UPK cohort over time. The results should be quantified to the extent possible. Additional pages may be attached as needed.

PART 2 – Instructions for completing the “11-12 UPK Sites” spreadsheet: Complete one line for each public school and/or community-based organization that provides instruction to UPK enrolled students.

Column A – List the name of the agency or organization providing the UPK instructional program.

Column B – Provide the address where the UPK classroom is located.

Columns C through V – In the column that best describes the organization providing the instructional program at that site indicate the number of UPK students enrolled in half day or full day classrooms.

For purposes of this report, the below definitions should be used by districts outside of New York City.

·  Day care center means a program licensed by the NYS Office of Children and Family Services (OCFS) to provide group child care services in an out-of-home setting.

·  Special ed 4410 means a program approved by SED to operate a special class integrated setting (SC/IS) program. In such settings, the UPK students generally are enrolled as the “typically developing” peers for the identified preschool children with disabilities who have Individualized Education Plans (IEPS) that prescribe placement is the SC/IS classroom.

·  Head Start means a federally-funded program serving children from families that are economically disadvantaged.

·  Nursery school means a program providing preschool services to groups of children in an out-of-home setting for less than three hours per day. Such programs are not required to be licensed or registered with the NYS OCFS.

·  Family/Group Family Day Care means a program, licensed or registered with the NYS OCFS to provide child care in a home setting to groups of 6-12 children.

·  BOCES means Board of Cooperative Education Services.

·  Nonpublic School means an organization, other than a public school, offering elementary or secondary education.

·  Museum means a prekindergarten classroom operated through collaboration with a museum on museum property and staffed by a teacher employed by the museum.

·  Library means a prekindergarten classroom operated through collaboration with a library on library property and staffed by a teacher employed by the museum.

Full day means that the UPK enrolled children are participating in a preschool experience of five or more hours funded either fully or in part by UPK grant funds. Where the UPK grant is used to fund a portion of the day and other funds, such as Head Start, are used to fund the balance of the day, thereby providing children with a preschool experience of five or more hours, the classroom would be categorized as full day.

Half-day means that the UPK enrolled children are participating in a preschool experience for a minimum of 2-1/2 hours, but less than five hours.
