***Must be completed BEFORE driving 15-passenger van/activity buses for the School District***

I have read School District 20’s Policy 2.1.3R for ‘Transportation of students for curricular and extra-curricular school events (
I have sent in my driver’s abstract information to the transportation department or my supervisor.
I have a class 4 (Unrestricted) license or better.
I have reviewed the following attachments:
  • ‘Hours of Service Requirements’ and reviewed the section on Log Books
  • ‘Hours of Service Requirements” and reviewed the section on Duty Time
  • ‘Vehicle and Air Brake Pre-Trip Inspections’
  • ‘Safety Guidelines for the Use of 15-Passenger Vans’
  • ‘Skills for Driving Buses, Taxis and Limousines’

Pre/Post Trip Inspection: I understand the requirements and know that these must be completed as per instructions.
Log Book: I understand the requirements and know that these must be completed as per instructions.
I understand I must turn in the original copy of my log book and pre-trip inspection to the transportation department upon completion of my trip.
I understand School Bus signs must be kept on the front and back of the van/activity bus.
Seat Belts: You and your passengers must wear seat belts at all times.
Traffic laws must be obeyed at all times. If conditions are not ideal, you may have to go much slower than the speed limit.
Road Conditions: Please drive to the conditions.
Bad Weather: Staying overnight is something that should be considered if the weather is bad. Communicate with your supervisor regarding this decision.
Overnight trips require a second adult to come along as a Chaperone in order to ensure hours of service requirements.
Gross Vehicle Weight (GVW): The weight of all passengers and luggage must NOT exceed the GVW. If you are unsure, use a weigh scale to double check.
Loading Luggage: Luggage must be evenly spaced in vehicle, secured so that it will not come free in the case of a sudden stop or accident.
Roof Racks: Must not be used at any time.
I understand that the vehicle must be cleaned after each use. This may be done either by the driver, or by the transportation department (charges will apply).
I understand I must report any tickets I receive from driving infractions while on this trip and may be responsible for their payment.
