1 On the Diabetes Education and Support page, please move the paragraph about Chiropody to the end of the content, after ‘For physicians’. And then after the Chiropody paragraph please add:

For healthcare providers including Diabetes Educators: find a referral form to the KDH Chiropody Clinic here.

Clicking on here brings up the Regional Chiropody Program – Referral Form (Nov 2012).pdf, which I will attach.

2 On the Outpatient Care page there is a change to the info under Chiropody. Please replace the line, “To access chiropody at KDH, please contact our Diabetes Educators at 613.258.6133, extension 424” with the following:

“To access chiropody at KDH, please speak with our Diabetes Nurse Educator at your Diabetes Clinic appointment, or ask your healthcare provider to fax a Referral to Champlain Diabetes Chiropody Services at 613.233.6713.”

And clicking on Referral brings up “Regional Chiropody Program – Referral Form (Nov. 2012).pdf

3 ON the Board of Directors page when I clicked on ‘Click here to see committee participation’, the document didn’t open up it just came into my computer as a download and I had to go to my downloads to see it. Can we make it open up when you click on it?

On the current board members sub-page

there is a shadow where the photo of Doreen Ulrichsen should be. I will resend the photo.

I also think these sub-pages (current and ex-officio members) could easily be overlooked. Can we put links to them at the end of the content on the main Board of Directors page?

4 On this page, please remove the grey box that says 490 x 250

5 On the Diagnostic Imaging page, I don’t think we need both the name of the test and the ‘Read More’ clickable. How about we make the names of the tests not-clickable? Jud, is that something we can do ourselves after launch? Also I would put the names of the tests in black not green – I’m not crazy about the green and the orange together.

Also, I really don’t like the Spotlight on Mammography there on the Diagnostic Imaging page. It detracts from the informative aspect of that page, going through the tests one by one. I was hoping you guys would think of a creative place to put that. Could you put it on the right as you did with the volunteer spotlight on this page: ?

On the mammo spotlight page, wherever it goes, at the end of the second paragraph, please change the Lean More to Learn More. And add a line before Watch CTV coverage of Mammography at KDH

6 Please make the same colour and clickability change referenced at the beginning of #5 above to the Education and Wellness page, unless I can do it myself after launch.

7 On the physicians and medical quality assurance page,

the photo of Dr Leonard seems ginormous. Could you please make it about half of that size? Again, if it’s something I can do, it can wait until after launch.

8 Change the pic on Careers/Volunteering, to Group Shot 062613 Styve.jpg

9 New images for homepage banner, with placement: if we count all the photos including the ones covered up by green, we have five per row. So if we started counting in the upper left with 1, and continue going left to right by row, we have photos of seniors in positions 1,2,3, 5 and 11. Let’s keep two of those (2 and 5) and replace 1, 3 and 11. Let’s replace 1 with bmd test, 3 with ER2, and 11 with knee surgery op.

10 On the News & Events main page,

it is just News (press releases) so maybe it should be renamed? Or maybe we should have some kind of overview on News & Events there, like we do on all the other section main pages…and then a separate news page?

Also the Jane Adams news story should not show up first in the list of press releases– it is incorrectly dated – please change the date on that to October 1, 2012

11 On the Events page,

the events are out of order and several have happened. Please do the following:

• Change date of KDH Foundation Spring Gala to March 28, 2014 and put it second last in the list

• Leave KDH Foundation Annual Tree Lighting

• Leave KDH Foundation ‘Kemptville Walks for Mammography’ but make it first in the list at October 05, 2013

• Change date of KDH Foundation Corporate Golf Classic to September 10, 2014 and make it last in the list

• Remove Diabetes and Your Feet

12 On the Education & Wellness events page, please remove all events except the Opening Airways session, and please change the line “Event Date: September 19, 2013” to read “September 19 – October 17, 2013”

13 On the Contact us page,

please do the following:

• replace the second line –Joe Buckle – with “Louis Guilbeault, VP Organizational Performance & Operations, ext. 259

• remove Boardroom

• remove the phone number for Dietary and Finance (2584997) – but leave the headings and the extensions that follow

•If you click on Directions in the left side navigation from the Patient/Visitor information section ( you get the page with the title Directions, but no google maps. Explorer users also seem to get nothing when they click on directions in the green drop down navigation under Patient/Visitor Information.
