1Introduction

1.1Funding should be sought prior to a planned admission for:

  • Patients covered under a block or cost and volume contract where there are additional costs associated with enhanced clinical needs over and above the existing package of care
  • Any patient treatment from out of the area

1.2This will enable funding to be sought before Northumberland, Tyne and Wear NHS Foundation Trust (the Trust/NTW) bears any form of cost.

2Process for Completion:

  • Medical Secretary/Clinical Lead to complete the form as soon as they have knowledge of a referral to their service
  • Identify which division and hospital
  • Enter allpatient information
  • Enter purchasing authority, if known (if this is left blank the Income and ContractedServices Accountant – Patient Care will complete based on Responsible Commissioner Guidance)
  • Enter all referring clinician details
  • Enter all dates and episodes of treatment to be considered
  • Sign, print and date the form
  • Submit to the Income and ContractedServices Accountant – Patient Care for approval
  • On receipt of approval from the CCG, notification will be made to the Secretary/Clinician to enable treatment to be arranged

2.1Once agreement is reached, the Income and Contracted Services Accountant – Patient Care will hold all the details until such time the corresponding activity shows on the monthly activity reports. At this point in time the agreement is used to generate the charge, making reference to the initial agreement.

2.2In Learning Disability (LD) services, it is quite normal for the Commissioner who has agreed to fund to be different to the Commissioner with whom the patient is registered with a GP. Placements are often jointly funded by Health and Social. This proves to be quite a difficult situation but strengthens the need to obtain funding agreement prior to admission.
