History:

Mr. X is a X-year-old male. Life seems to “suck” now. People can’t take “your crap” anymore, especially your boss. Sometimes you’re sharing needles. You stopped going to your Adult Ed classes, your friends dropped you and so did your girlfriend because you never do anything with them. Your girlfriend is drug-free and never touched the stuff. Your girlfriend is the only person who has really gotten you and understood why you are this way. You are “dope” sick and don’t know what to do anymore. You are being straight with the provider and not creating a story about fake pain. Has thought about the bad stuff heroin was doing to you, which only made you want to use more.

Drug Screen:

You use heroin intravenously every day, about 4-5 bags a day throughout the day.

Negotiation:

On a scale of 1-10 you choose a 5.

Reasons to change:

You have no place left to get stuff and are worried that you will just get more and more sick. Also, you are tired of this pattern. You would like to slow down a little or take a break on some days. You miss your girlfriend, but your girlfriend said she will take you back if you got into treatment.

Cons:You like using, it makes you feel good.

Research Interventionalist

History:

Mr. X is a X-year-old male. His drug use has been affecting his life. Patient reports his life seems to “suck”, people can’t take “his crap” anymore, especially his employer. Sometimes he shares needles. He stopped going to Adult Ed classes. His friends dropped him and so did his girlfriend because he never does anything with them. His girlfriend is drug-free and never used. She is the only person who has really “gotten” him and understood why he is this way. Patient is “dope” sick and doesn’t know what to do anymore. Patient reports being “straight” (honest) with the provider about drug use and claims not to be creating a story about fake pain. Patient has thought about the bad stuff heroin is doing, but that makes him want to use more.

Drug Screen:

Patient uses heroin intravenously every day, about 4-5 bags a day throughout the day.
