SUPERINTENDENT EVALUATION FORM

The Superintendent will rate their own performance on each evaluation item. The numerical ratings must be supported by comments giving rationale and as much objective evidence as possible. Following its completion, a copy of the evaluation shall then be given to each member of the Board and they will rate the superintendent on each evaluation item.

Board member ratings and responses should be fair and objective, based upon first hand knowledge and experiences. It is the duty of all participants to be honest and forthcoming to ensure the evaluation process is a positive tool to be used for improving performance, clarifying expectations, and improving board/superintendent relationships.

The following criteria shall be the basis for determining numerical ratings:

4 – Demonstrates exceptional level of competency;

3 - Demonstrates effective level of competency;

2 - Demonstrates adequate level of competency, but needs improvement;

1 - Demonstrates unacceptable level of competency and needs substantial improvement.
