YOU ARE NOW COMMANDED to enter into the rectory [or vicarage] and parish church of (place) and take and sequester them into your possession and to hold them in your possession until from the rents, tithes, rentcharges in lieu of tithes, oblations, obventions, fruits, issues and profits thereof and other ecclesiastical property in your diocese of and belonging to the rectory [or vicarage] and parish church of (place) and to the defendant (name) as [rector][vicar][priest in charge] thereof you shall have made the sum(s) of £ and £ for costs of execution and also interest on £ at the rate of £ per cent per annum from the (date) until payment and that immediately after execution of the writ to pay the claimant (name) in pursuance of the said judgment [or order] the amount levied in respect of the said sums and interest.

YOU ARE ALSO COMMANDED to indorse on this writ immediately after execution thereof a statement of the manner in which you have executed it and send a copy of the statement to the claimant (name).
