Worksheet for Identifying Possible IIS and Other Public Health Activities for CMS Funding

Begin by identifying all the areas for which you could use additional funding for your IIS. For each candidate activity:

  • Provide a short-hand name for the activity, using your customary IIS terms; e.g., “Provider on-boarding,” “test and deploy bi-directional exchange,” etc.
  • Re-state the activity in terms that are most meaningful for Medicaid; e.g., “Increase IIS capacity to accelerate on-boarding of Medicaid providers for stage 3 Meaningful Use,” “test and deploy bi-directional query-response capabilities to ensure IIS readiness for stage 3 MU requirements,” etc.
  • List any new staffing requirements to achieve that activity, including a rough budget estimate.
  • List any new software enhancements or new procurements, as well as any hardware upgrades/procurements needed to meet that activity, including a rough budget estimate.
  • Finally, provide a justification for why you believe this is an appropriate activity to include in an IAPD. This is to ensure you fully integrated the Medicaid perspective in your thinking and language (see Module 2 for more on this).

Repeat as many times as you have activities, or at least until you believe you are ready to have concrete, productive and credible initial conversations with Medicaid when they ask, “What are you looking for? How much would that cost?”

Avoid putting too much effort into any cost estimates until you have a better sense of the willingness of your Medicaid partners. It’s good to have ballpark figures ready to go but, at the same time, you don’t want to have wasted hours on detailed estimates for an activity Medicaid rules out in your first meeting.

As you estimate costs for our candidate activities, keep the 10% match and the fair share cost allocation principle in mind (see module 1); CMS will not cover 100% of these costs, which means your program and/or agency must identify appropriate funds for the remainder. There is undoubtedly more money on the CMS/Medicaid than on the public health side of the equation, so don’t get too far ahead of the resources available to you on the public health side. You may also need to cover all costs until the first reimbursement payment from CMS arrives at the health department.
