Definitions:
Administrator, the manufacturer of PermaPlate products, shall mean Siskin Enterprises, Inc.
Warranty shall mean this Limited Warranty Registration Form.
Warranty:Administrator herebyguaranteestothevehicleownerthatsuchvehicletreatedwithPermaPlateproductswillnotsustain damageasdescribedbelow.Thisguaranteeiseffectiveforthedurationof Warrantyasperthetermselectedonthereversesideofthiscertificate.This warranty applies only to areas of the vehicle where proper application has been completed and does not cover damage existing prior to product application.This Warranty is a LimitedWarranty andissubjecttoalltermsandconditionsassetforthonbothsidesofthisdocument.
Right to Transfer:Theabove vehicle ownershallhavetherighttotransferthisWarrantytothenextownerofthisvehiclefreeof charge.Tocompletesuchtransfer,Administrator mustbenotifiedofthechangeinownershipwithinthirty(30)daysfromthe dateoftransfertosuchfirstsubsequentowner.
WINDSHIELDPROTECTIONPROGRAM (WPP)
PermaPlateWindshieldProtection:Should the front windshield that has been treated with the PermaPlate Windshield Protection Product become damaged by propelled rocks or other propelled road debris while driving on paved roadways, causing cracks and chips, Administrator will pay for the repair of the damage portion of the windshield. If Administrator’s certified technician determines that the covered windshield damage cannot be repaired, Administrator will allow for the damaged windshield to be replaced. Administrator reserves the right to replace the windshield with like kind and quality glass. If like kind and quality glass is not available. Administrator will replace the windshield with Original Equipment Manufacturer (OEM) glass. Coverage under this Warranty shall not exceed an aggregate total of $5,000.00 over the lifetime of this contract.
Definitions: Luxury Vehicle coverage applies to all vehicles manufactured by Acura, Audi, BMW, Cadillac, Hummer, Infiniti, Jaguar, Land Rover, Lexus, Lincoln, Mercedes-Benz, Porsche, SAAB and Volvo; as well as specifically the Chevrolet Corvette and Dodge Viper. Coverage under this Warranty for luxury vehicles shall not exceed an aggregate total of $5,000.00 over the lifetime of this contract. Luxury Vehicles are eligible for luxury coverage only. Standard Vehicle coverage applies to non-luxury vehicles and shall not exceed an aggregate total of $1,200.00 over the lifetime of this contract or $600.00 for any one claim. Standard Vehicles, however, may upgrade to Luxury coverage at the time of purchase.
LimitsofCoverage:PermaPlateWindshieldProtectionmay ONLYbepurchasedatthetimeofvehiclesaleoruponinstallationofanewwindshieldwith proofofinstallationbyanauthorizeddealer.Administratorwillnotcover:pre-existingdamage,cracksorotherdamagescausedbyanyperilotherthantheimpact ofsmallrocksorflyingroaddebris. Examplesofexcludeditemsinclude,withoutlimitation:stresscracks,pitting,collisionorsimilaraccident,inclement weatherconditions(i.e. lightning,earthquake,hailstorm,sandstorm,etc),factorydefects,damageduetotheft,vandalism,actsofwarorterrorism,damageoccurringwhiledrivingoffpavedroadways,orwindshieldreplacementnecessitatedbyfailureof thevehicleownertotakereasonablestepstomitigatefurtherdamagefromoccurring. RV’s, motorcycles and exotic vehicles are not eligible for this Warranty. Exotic vehicles include, but are not limited to Alfa Romeo, Austin Healy, Aston Martin, Bentley, Bugatti, Checker, Citroen, Daihatsu, Ferrari, Fisker, International, Lamborghini, Lotus, Maserati, Maybach, McLaren, MG, Panoz, Peugeot, Rolls Royce, Saleen, Shelby, Tesla, Triumph, and TVR.
RepairObligations:Ifwarrantycovereddamagehasoccurred,Administratorwillpayonlyfortherepairofthedamaged portioncovered damageofthevehicle.Administratorreserves therighttoinspectforliabilityormayrequestestimatesforrepairand/orphotographsofthedamagetodeterminethevalidityoftheclaim. Administratorhas sole discretion in determining and implementing repair procedures. Such repairs will be performed with reasonable promptness and with quality workmanship.Administratorhasnoobligationforreimbursementoftransportationorinconveniencecostsduringtimeofrepair.Administrator’sobligationshallbelimitedtothelesserof:thebalanceremainingoftheaggregatetotal;theper incident maximum for standard coverage as defined above; the repair;or,ifnecessary,theremovalandreplacementofthewindshieldwithOEMorlikekindqualityglass.
ClaimProcedure:Tobevalid,aclaimmustbefiledduringthewarrantytermselectedandmayonlybepaidonaproperlyregisteredand,ifapplicable, properlytransferredWarranty.Inordertoreasonablyminimizefurtherdamagewhichmightoccur,aclaimalsomustbefiledwithinfourteen(14)daysfromtheearlierofeithertheappearanceofdamagecoveredbythisWarrantyorthetimewhendamage could have been discovered upon reasonable observation or inspection by contacting Administrator at the telephone number, address or web site listed below. FAILURETOPURSUEACLAIMWITHINSIXTY(60)DAYSFROMTHEONSETOFDAMAGEVOIDSTHEWARRANTYPERTAININGTOSUCH DAMAGE. ANYREPAIRUNDERTAKENWITHOUTEXPRESS,WRITTENAUTHORIZATIONFROMADMINISTRATOR WILLNOTBEELIGIBLEFOR REIMBURSEMENT.
SISKINENTERPRISES,INC.IS THEPROVIDEROF THISPROTECTIONPRODUCTGUARANTEE.TERMSANDCONDITIONSAREASSTATED ABOVEANDCANNOTBEALTERED. THISGUARANTEEISAPRODUCTGUARANTEEANDISNOTINSURANCE. ThePermaPlateproductsare permanentlyappliedtothecoveredvehicle;thereforethisprotectionproductguaranteeisNON-CANCELABLEandNON-REFUNDABLE.Thisprotection productguaranteespecificallyexcludesSiskinfromliabilityforincidentalorconsequentialdamagescausedbyuseoftheseproducts.Somestatesdonot allowtheexclusionorlimitationofincidentalorconsequentialdamages,sotheabovelimitationorexceptionmaynotapplytoyou.Noexpressrightsare givenunderthisprotectionproductguaranteeexceptforthosespecificallydescribedherein.Thisprotectionproductguaranteegivesyouspecificlegal rightsandyoumayalsohaveotherrightswhichvaryfromstatetostate. Obligationsoftheprotectionproductguaranteeproviderunderthisprotection productguaranteeareinsuredunderareimbursementpolicy.IfcoveredserviceisnotprovidedbyGuaranteeproviderbeforethe60th dayfromdateof valid claim initiation, vehicle owner may apply directly to Wesco Insurance Company (a member of AmTrust Group), 59 Maiden Lane, 43rdFloor, New York, NY 10038 for benefitsaffordedunderthisprotectionproductguarantee.
