When you are injured and minutes are critical, CALSTAR (California Shock Trauma Air Rescue)

your local non-profit, community service, air ambulance is there to provide rapid air transport

and lifesaving medical care. Medical research has shown that critical patients, who receive

trauma treatment within the “golden hour” of an accident, have greater chances of survival and

recovery. CALSTAR delivers critical patients by helicopter to the closest appropriate trauma

hospital, with in flight patient care provided by two certified Critical Care Registered Nurses.

CALSTARpilots have 3,000+ flying hours and have the experience to fly and land in remoteareas.

A Lifesaving Flight Can Be Priceless

Your recreational activities take you to remote locations where emergency medical resources are

limited in the event of an emergency. While we hope you never need to fly with CALSTAR, we

understand the financial ramifications of an air ambulance transport. Even insured patients may

be left with many thousands of dollars in out-of-pocket expenses. We understand that during a

medical crisis, the incurred medical costs can be burdensome. However, as a member of

CALSTAR, we will bill only your insurance plan for our services. While CALSTAR flies anyone in

need, as a member of CALSTAR you will not be billed for our services, even if you do not have

health insurance.

Tahoe Rim Trail Endurance Run Group Membership - Special Offer

CALSTAR frequently transports patients who suffer injuries while participating in endurance and

recreational events. Understanding the remoteness of the run and the risk factor, CALSTAR

would like to offer a Tahoe Rim Trail Endurance Run (TRTER) Group Membership to run

participants and volunteers. The cost for CALSTAR’s annual group membership is only $35.00 per year for a Family or Individual Membership. CALSTAR will waive the14-day waiting period for TRTER participants. If you suffer a qualified medical emergency andCALSTAR or one of our reciprocal partners provides air rescue, you will not be billed for airmedical transportation.

CALSTAR’s Annual Membership Program

CALSTAR has nine bases, which are located in Auburn, South Lake Tahoe, McClellan Park,

Concord, Gilroy, Ukiah, Salinas, Santa Maria and Oakland, California.Your CALSTAR

membership covers services provided by any of our bases. Through AAMMP (Association of Air

Medical Membership Programs), CALSTAR membership includes reciprocal relationships with

other nonprofit air ambulance providers. This means that your membership coverage travels with

you to Northern Nevada, Oregon, Idaho, Washington & Western Montana. If one of our

partner air ambulances transports you, they will honor your CALSTAR membership.

INSTRUCTIONS

Please complete the attached Membership Agreement and payment option. Checks should be

payable to CALSTAR. If purchasing a Family Annual Membership, please list family members

and birth dates. If paying by credit card, please include complete account number, expiration

date, and name on credit card. Lastly, please read the membership agreement and sign your

name as acknowledgement. Note the membership is only valid with payment and the required
