The transition action plan

This document should be completed prior to the young persons 17th birthday, and should be updated where necessary before the young persons 18th birthday and their transfer to adult services. In it you should record what it is that the young person wants to achieve for their own futures what are their hopes and aspirations.

This isn’t always easy for us to do and sometimes we will have to rely on what the people who know them well tell us about what their views are. For young people who don’t use words as their way of communicating we are going to be listening to the people who love and care for them about what they see and say.

The plan should also look at what are the achievements of the young person, what work you as a worker have done and what issues need to be carried forward and looked at as the young person grows up in to early adult life.

So in thinking about the future what do we know about the young persons

  1. Education and training
  2. Employment.
  3. Money- are they getting all of the money they are entitled too.
  4. Family support and friends
  5. Important friendships and relationships
  6. What do they like to do in their free time what would they really like to try in the future.
  7. Housing and were the young person might want to live in the future.
  8. Transport and independent travel

It is also the place were you can summarise your view and make recommendations for future work with the young person and their family or carers.
