Team Member Agreement
Note to recruiter:This agreement was created to ensure that all team members are working together in the best interest of the child. It can be tailored to include team members within each jurisdiction. The agreement should list all team members involved and their roles. A copy of the initialed agreement should be given to each team member for every child on the case.

Child-focusedrecruiter: I believe that every child deserves a permanent and loving family. I understand that my role is to:get to know each child that I work with and ensurethat each child is prepared for adoption; recruit a forever home by looking into a child’s past and current connections; identify matches, and prepare potential matches for transition and adoption. I understand that I am in a support role to the (NAME OF CUSTODIAL AGENCY), not leading the case direction, and operate under the authority of the decisions made by (NAME OF CUSTODIAL AGENCY). I also agree to promote a spirit of teamwork and cooperation in my workplace and community, and always encourage the team to act in the best interest of the child.

Case worker: I will give the child-focused recruiter access to a child’s case file (electronic and hardcopy) and minimum monthly contact with the child. I will maintain minimum monthly contact with the recruiter and inform the recruiter of critical incidents, team decision meetings, court dates, and reviews. I will provide timely feedback for proposed recruitment activities and matches. I will give the recruiter broad support to prepare the child for adoption and recruit for an adoptive home. This includes using fliers, narratives, and video footage with prospective identified matches, after my approval.

Adoption case worker: I will assure the recruiter has access to all parts of a child’s case file. I will consistently participate in meetings with the recruiter to assess progress and help resolve any difficulties that affect permanency. I will schedule and participate in an initial meeting between the recruiter and the recruitment team, with possible future meetings if needed. I will include children on the recruiter’scaseload in my agency’s general adoption recruitment efforts, including, but not limited to: (GENERAL ADOPTION RECRUITMENT EFFORTS). I agree to inform the recruiter of potential matches in a timely manner.

Other team members(supervisor, therapist, Guardian Ad Litem, foster parents, school personnel, etc): As an active team member, Iwill keep in regular contact with the recruiter and give feedback about adoption preparation, recruiting efforts, and transition planning, while encouraging the team to act in the best interest of the child.
