Welcome to CCSNH Open Enrollment for 2015

Open Enrollment for Medical, Dental, Medical Buy-Out, and Flexible Spending Accounts will be held November 3 – November 24, 2014

Open Enrollment is the time each year when you have the opportunity to review your CCSNH benefit elections and make changes. During Open Enrollment, you will be able to:

  • View your current benefit elections and benefit plan information.
  • Make changes to your Medical, Dental, and Life Insurance coverage. Please note: If you are not a new hire, all Life Insurance enrollments will be subject to Evidence of Insurability.
  • Enroll or re-enroll in the CCSNH Medical Buy-Out (MBO) program. The MBO provides benefit eligible employees an incentive for waiving their medical insurance.
  • Enroll or re-enroll in Healthcare and/or a Dependent CareFlexible Spending Account (FSA).*
  • Enroll in a voluntary/optional benefit plan - DeltaVision and/or employee, spouse, or dependent life and AD&D insurance. *
  • Verify dependent information and add or remove dependents from coverage.
  • Review and, if necessary, update your insurance beneficiary (ies).

* Employees are responsible for premium costs and administrative fees associated with voluntary/optional benefit plans. The FSA monthly administrative fee is $3.65 per account, which is paid through bi-weekly payroll deductions.

Go to CCSNH Employee Self Service Portal at to begin the open enrollment process. Enter your “log in” information and click “Benefits > Health & Welfare.” For assistance in accessing the self-service portal, go to the CCSNH Benefits web site, Click on ‘Health and Welfare Benefit System – Online Benefit Enrollment system’ and follow the step by step instructions.

Medical: CCSNH offers two (2) medical plans- Anthem BCBS Lumenos HRA BlueChoice New England Plan (“BlueChoice NE”) or Anthem BCBS Lumenos HRA National PPO Plan (“National”). Effective January 1, 2015, the bi-weekly employee contribution for each medical based on 26 pay periods is as follows:

Important: If you wish to continue with your current medical plan, you do not need to make any changes to your medical plan election.

Delta Dental: Delta Dental now offers EyeMed, an additional discount program for eye wear, for employees and their dependents who are enrolled in the CCSNH dental plan.
