During my absence Bo’s Place, 35 Llysfaen Road, Old Colwyn, Colwyn Bay LL299HB, will be responsible for my pet(s). In the event of an emergency I authorise the above-named veterinary practice to provide any medical treatment that may be necessary. If, for any reason, this surgery is unable to provide the treatment, I give permission to Bo’s Place to transport my pet(s) to an alternative veterinarian. I agree to be responsible for all charges, including, but not limited to, vet fees and transportation costs.

I understand that Bo’s Place will be free from all liability relating to transportation to and from veterinarian and treatment for sickness or emergency. I have read and understand the terms and conditions (shown overleaf) of boarding at Bo’s Place.

TERMS AND CONDITIONS

Bo’s Place welcomes only non-aggressive, well-behaved dogs over 12 months old.

Dogs must not have been in any other kennelling establishment within two weeks of boarding at Bo’s Place.

Dogs must not have any history of having attacked other animals or people/children.

Dogs must be clean and free of ticks/fleas and must have completed anti-worming treatments.

Any dog demonstrating uncontrollable or dangerous behaviour will be removed to kennels at the dog-owner’s expense plus an additional transfer fee of £25 will be payable.

In the event of a dog not being collected at the end of the boarding period, Bo’s Place reserves the right to re-home the dog by any reasonable means.

No dog registered under the Dangerous Dogs Act 1991 will be accepted for home boarding.

Bitches in season, or due to be in season during the boarding period are not accepted.

Boarding fees are per day. Failure to collect your dog by 10 a.m. on the final day of boarding will incur additional charges at £10 per hour.

Cancellation less than fourteen days prior to the agreed boarding period will result in loss of deposit.

Vaccinations: Proof must be provided that dogs have current vaccinations against Canine Distemper, Infectious Canine Hepatitis (Canine adenovirus), Leptospirosis (L. canicola and L. icterophaemorrhagicae) and Canine Parvovirus and other relevant diseases. The course of vaccination must have been completed at least four weeks before the first date of boarding or in accordance with manufacturers’ instructions. The vaccination record must be left with Bo’s Place for the duration of the boarding.

In the event of your dog requiring veterinary treatment while at Bo’s Place the owner will undertake to pay all resulting fees, which will include £15 transfer fees for each veterinary visit.

Bo’s Place will not accept any liability for loss, injury or death to the owner’s dog(s) unless (and only to the extent that) such loss, injury or death is recoverable under the terms of the insurance policy.
