INSTRUCTIONS

  1. Use separate Form for each class/year.
  2. This form should invariably be signed by the Principal of the college/authorised person (the person so authorised should not be below the rank of Professor).
  3. Only eligible students are admitted to the course concerned. Responsibility for making wrong admission will lie entirely with the College authorities. The college shall have to pay a penalty of Rs. 1,00000/- per candidate admitted wrongly. Further, expenses on any litigation arising between a student and college, out of wrong admission, where Baba Farid University of Health Sciences is made a party in any court of Law/Tribunal shall be borne by the college concerned, including those incurred by Baba Farid University of Health Sciences.
  4. A soft copy of the return should be send on floppy disk/CD-ROM (using Excel File format).
  5. The Principals should submit within 10 days of cut off date of admissionpositively the registration returns with the following documents in original :-

  7. Certificate of 10+2 examination/ Degree in case of PG/Diploma courses.
  8. Certificate of 10+1 examination
  9. 10+1 & 10+2 regular studies certificate.
  10. Matriculation Certificate.
  11. Character Certificate from last attended institution.
  12. Migration Certificate from the last attended University/Board/Council.
  13. Entrance Test pass certificate.
  14. Gap year self undertaking (if applicable).

NOTE: Fine for late submission of required documents (to be submitted with registration Return) is as under :

  1. The spelling of the names, father's/mother's name and date of birth, noted in this return are in accordance with the particulars entered in the relevant gazettee/original certificate.
  2. The students admitted are eligible and fulfil the conditions of percentage of marks laid down under the regulations in force at the time of admission to the course concerned and all admission have been made strictly in order of merit of Entrance Examinations except NRI candidates who are admitted on their inter-se-merit.
